Job summary The Optalis Short-Term Support and Rehabilitation Team (STS&R) provides reablement care and support to people who have recently been discharged from hospital.
Reablement is a short-term, goal-oriented support service aimed at helping people regain their independence after an illness, injury, or hospitalisation.
Instead of doing tasks for someone, reablement professionals work with individuals to build or rebuild skills so they can manage daily activities on their own, such as dressing, cooking, or moving around safely.
The focus is on empowering people to maintain as much independence as possible, improving their confidence and quality of life.STS&R is a multi-disciplinary team consisting of Occupational Therapists, Physiotherapists, and Rehabilitation Care Assistants providing support to customers across the Royal Borough of Maidenhead and Windsor.
Main duties of the job We are looking for a qualified Occupational Therapist who is registered with HCPC and has experience of rehabilitation and supporting people in the community.The aim is to support the local NHS communities team with discharging patients back to their own homes and into local care settings.
This role will including assessing patients and living conditions, providing equipment, guidance and making suitable adjustments to allow them to continue living as independently as possible.
You'll work alongside our team of OTs and Physiotherapists, as well as support staff from the NHS, to help agree a person-centred support plan that can be delivered by our team of Rehabilitation Care Assistants.The role is based out of Maidenhead Town Hall and will include working in the community, visiting customers at various locations across the borough.
It is essential that you are able to drive and have your own vehicle.As our customers receive a maximum of six weeks support, your caseload will be varied and will change regularly, truly making no two weeks the same.

Job description Job responsibilities Primary Purpose of the Role;To work specifically within our Integrated Discharge and Flow Pathways in the postDischarge space.To work alongside Carers, Physiotherapists, and other members of the team to maximisepotential and establish long-term needs in the first few weeks post discharge.To advocate for the resident and ensure needs and wishes are represented workingclosely with family and significant others.To work with people to maximise potential and endeavour to keep residents in their ownhomes.To work with residents in Care homes where placement is interim, with the aim ofsupporting and enabling residents to return home where possible.To provide day to day ongoing professional support to residents as required and wherepossible, promote social change.Monitor and review Occupational Therapy outcomes in line with policy and procedures.Assist in multi-disciplinary problem solving to enhance wellbeing using relevant professionalskills.To work in an inter-disciplinary way with the rest of the team, understanding their roles anddeveloping close integrated working ways.
