The ICT is a multi-disciplinary team that's aim is to prevent avoidable hospital admissions and reduce delayed hospital discharges by assessing needs and providing rehabilitation with/without care support in the home setting.As part of this multi-disciplinary team, the post holder will manage their own caseload and work alongside colleagues to ensure seamless service delivery.
