Join our dynamic Integrated Care Service (iCares) team as a Band 6 Occupational Therapist!
You will play a crucial role in delivering specialist clinical care within a community-based Virtual Ward , providing rapid response assessments and intensive interventions in care homes.

Key Responsibilities: Manage a clinical caseload requiring rehabilitation.
Provide comprehensive assessments for patients with complex needs.
Develop tailored treatment plans based on advanced clinical reasoning.
Supervise and support junior staff and students.
Contribute to service improvement projects and clinical audits.
Ensure effective communication with patients, families, and multidisciplinary teams.
Participate in discharge planning and follow-up care as needed.
About You: Qualified Occupational Therapist registered with the Health and Care Professions Council (HCPC) .
Experience working in a fast-paced clinical setting, particularly in community care.
Strong communication and teamwork skills.
Ability to lead and mentor junior colleagues.
