3 months ongoing contract with Local government Job Summary: Within the context of selfdirected support personalisation and working as a member of a multidisciplinary team responsible for meeting the assessment care management and support needs of vulnerable adults the post holder will: Manage an independent caseload assessing people with physical disabilities/ sensory impairment/ mental ill health/ learning disabilities.
Take specific responsibility for the provision of appropriate equipment / minor and major adaptations / assistive technology and the planning/implementation of appropriate reablement programmes.
Key Duties (Sample): To contribute to effective support planning to enable citizens and carers to make their own decisions by exercising choice encouraging independence and access to community resources to meet their assessed needs.
To facilitate and contribute to the assessment of the needs of vulnerable adults and their carers feeding into the process of specialist and more complex assessments in line with the principles of selfdirected support and single assessment.
To support citizens to promote their health wellbeing choice and control.
To visit citizens in their own homes (with other professional staff when required) and assess their potential and needs for equipment/minor adaptations/assistive technology.
To demonstrate the correct use of equipment and to ensure the recipient is confident in its use.
To be responsible for ordering and ensuring timely delivery of equipment from the Integrated Community Equipment Service.
To review equipment provision with the service user and ensure all needs have been met.
To ensure the provision of comprehensive case records assessments and reports on service users.
To work closely with other members of the multidisciplinary team.
To work flexibly with other colleagues from other disciplines and other community agencies in enabling people to access community resources lead independent lives and make their own decisions.
To participate in individual and team development activities.
To take part in any training identified as necessary for the correct performance of the job.
