Job summary The purpose of the post is to deliver safe, highquality primary care nursing services to the practice population.
Workingwithin the NMC Code of Conduct and utilising primary care skills andexperience, the post holder will provide advanced nurse interventions andinfluence development of nursing services.
Main duties of the job The ideal candidate will demonstrate critical thinking in the clinical decision-making process.
They will work collaboratively with multi-professional teams to meet the needs of patients, supporting the delivery of policy and procedures.
In order to work at this level, NMC requirements for advanced practice must be met.In summary, the post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.

This encompasses the provision of evidence based, high-quality care for patients, whose issues fall within urgent/acute episodes and including health promotion and public health elements.
Our nurse practitioners undertake sessions of either telephone calls with patients or contact with patients using our digital platform.
Job description Job responsibilities Supporting the overarching Lakeside Healthcare Group strategy and service delivery, the post holder will provide complete episodes of care for patients with a wide variety and range of presenting problems and health care needs.
This encompasses the provision of evidence based, high-quality care for patients whose issues fall within urgent/acute episodes, long-term chronic conditions and including health promotion and public health elements.Working within the NMC Code of Conduct HCPC Standards and utilising primary care skills and experience, the post holder will provide advanced care interventions.
Using evidence-based knowledge, the post holder will deliver a high standard of care, making professionally autonomous and accountable decisions.
The post holder will work within their professional boundaries to demonstrate safe clinical decision-making and cost-effective, expert care for patients within the practice.The post holder will support service development, the safe delivery of policy and procedures and demonstrate effective nurse/AHP advocacy and leadership.Clinical responsibilities:An ability to make professional evidence-based decisions for which they are accountableAn ability to assess, diagnose and treat patients with undifferentiated and undiagnosed problems.
Screen patients for disease risk factors and early signs of illnessMake differential diagnosis using decision-making and problem-solving skills in conjunction with senior medical and nursing colleaguesDevelop, with the patient, an ongoing care plan with emphasis on health education and preventative measuresOrder and follow up on necessary investigations providing treatment and care both individually, as part of a primary care team and through referral to other agenciesWork collaboratively with other health care professionals and disciplinesParticipate in assessment and care planning of patient care liaising with GP and other AHPs as requiredImplement and evaluates care and treatment in agreement with the patient and as defined in the care plan escalates any deviation from the care plan or change in condition to the Lead GP accordinglyCommunicate effectively with patients, carers and all members of the multidisciplinary team.Participate in MDT meetings within PCNs and affiliated bodiesActively participate in clinical supervision and support with assigned GP mentorPrioritise daily work in line with clinical patient needImplement and work to ensure evidence based care best practiceInitiate health promotion activities with individuals, patients and carersAn ability to provide leadership, education and mentorshipAn ability to provide clear and accurate documentation An appreciation of the importance of confidentialityThe ability to assess for safeguarding issues and proactively refer on to relevant organisations / safeguarding teamsAn acknowledgement of the importance of equality, diversity and the rights of patients, carers and colleagues.
