Job summary Vine Medical Group is recruiting for a full or part time NursePractitioner / Advanced Nurse Practitioner to join our Same Day Care Team.
Our new team member will work both independently and withina multi-disciplinary team environment, to provide safe, holistic care to ourpatients.
This will include remote and face to face appointments, reactive andproactive home care visits.
You will berequired to work within your professional competence and boundaries to provideboth urgent and planned care to patients.You will need to prioritise and triage the needs of patients, making anynecessary referrals for investigations or to secondary care in the appropriatemanner.This role is particularlysuited to an autonomous clinician with a proven track record of delivering highquality care within the Primary Care setting with experience in dealing with awide range of minor ailments.Please note that we reserve the right to close applicationsfor this vacancy early if there is a high volume of applications.We encourage and welcome applications from veterans andservice leavers.
Main duties of the job To deliver high quality clinical care in a timely manner asappropriate to the patient needs.
To provide a first point of contact within the service forpatients presenting with undifferentiated physical and mental healthconditions, offering holistic assessment.To include history taking, physical assessment, undertakinginvestigations to inform clinical decision-making to establish a diagnosis andmanagement plan in collaboration with the patient and the families or carers.

Job description Job responsibilities This role involves workingautonomously to a high standard of care within the same day care team triaging,assessing, diagnosing and treating minor illness and ailments as part of our onthe day service, both on-site and at home.
Supply, administer and prescribemedicines as indicated (and appropriate) to address patient need, according tomedicines management policy, patient group or patient specific directions orNMC/HCPC standards.Provide telephone or videoconsultations, including the safe and effective triage of acutely unwell patients.Conduct reactive home visits tothose acutely unwell patients, assess, plan, treat and arrange admission tohospital if required, for the practice and within the PCN.Conduct proactive home visits tohousebound patients with long term conditions, including but not exclusivelydementia, diabetes, respiratory disease.
Provide advice to patients andtheir families or carers on general health care, lifestyle, advice, referringto internal or external services.
Ensure clinical practice is safe,effective and evidence based and remains within boundaries of competence andacknowledges limitationsto work within a team, be part ofand to create a positive culture and atmosphere for patients and staffMake appropriate referrals toprimary, secondary and social care services, following practice (and/or locallyagreed) pathways and protocols.
Promote self-care and educateservice users to allow them to be empowered to make informed choices abouttreatment.Understand local safeguardingreferral process and be aware of appropriate referral mechanisms for childrenand vulnerable adults Person Specification Qualifications Essential Registered Nurse Practitioner / Advanced Nurse Practitioner with NMC Qualified to Master's degree or equivalent Full UK Driving Licence Physical Assessment and Minor Illness Training Desirable Accredited training in any of the following: Urgent care, Chronic disease management or frailty Non-Medical Prescriber Knowledge & Skills Essential Clinical examination skills across a diverse range of patients including both children and adults Able to build positive rapport with a diverse range of patients and colleagues Effective communicator, able to communicate complex and sensitive information to patients and carers, recognising the need for alternative methods of communication to overcome differing levels of understanding, cultural background and preferred ways of communicating, whilst ensuring the patient is fully informed Able to work autonomously within your scope of practice whilst also having an understanding of when to access the wider clinical team Desirable Working knowledge of EMIS and Docman medical systems Experience Essential 3 years+ experience of working as an Nurse Practitioner / Advanced Nurse Practitioner Background of working in a fast-paced Primary Health Care or acute environment is essential Proven record of acting in roles where you have acted autonomously and responsibly for the assessment, treatment and/or appropriately referral patients to specialist services when required
