The Intermediate Care Service supports medical and surgical service users from discharge from hospital, to their journey home.
Every day there is something different from complex wound care, catheter care, phlebotomy, to monitoring deteriorating patients and so much more in between.

The Nursing Team boasts a variety of skills and experiences to enhance your knowledge and professional development within a supportive environment, we pride ourselves on excellent standards of Nursing Care and service user satisfaction.
The Nursing Team works collaboratively with the multi-disciplinary team compromising of, Occupation Therapists, Physiotherapists, Pharmacists, Healthcare Support Workers, a medical team and our Kirklees Care Team.
Together we rehabilitate service users to succeed in their home environment, preventing hospital admissions and preserving safety.
We offer a 7-day service, working 8am until 7pm in a rota pattern providing assessment, treatment and evaluation of clients' needs using a holistic and patient centred approach.
The ultimate aim being to prevent unnecessary future admission to hospital and to ensure the swift appropriate discharge from hospital to offer rehabilitation and enable our clients to return home.
The team will also work with partner services, with the aim of avoiding premature admission to long term care.
Working with initiative is essential, and there will be opportunities to support and skill share with colleagues within the team as well as across the different sectors.
