This individual requires specialised clinical care due to several complex needs.
They have a tracheostomy, which requires ongoing respiratory management, and experience mobility issues, necessitating the use of a ceiling track hoist for safe transfers.
They are minimally conscious and struggle with communication, requiring additional support for interaction.
The person also benefits from access to a therapy room and requires regular physiotherapy to support their physical health and mobility.

SHCCJM Responsibilities Work closely with the Nurse Manager Team to provide quality care to an adult client Commit to a regular working pattern and collaborate with Care Coordinators for effective Provide 2-2-1 support to an adult with tracheostomy, ceiling tract hoist and physiotherapy.
