Job summary An exciting development opportunity has arisenfor a dynamic and motivated Registered Nurse to join our Early assessment team (EAT) as a band 6 Nurse Coordinator.Working hours will cover8am-9pm over 7-days, including Weekends and Bank Holidays.
This position willbe based at Pinderfields Hospital.Thisis an excellent opportunity for a self-motivated and enthusiastic person tojoin Mid Yorkshire Teaching NHS Trust.
As a Nurse Coordinator you will be working collaboratively withOccupational Therapist, Physiotherapist, Social Workers, and Acute Hospitalstaff to prevent unnecessary hospital admission at the Front Door, tofacilitate earlier discharges and help people to get the support that isrequired to help them be cared for in the place they call home.Experienceis beneficial with knowledge of community services, and an interest indischarge and patient flow.Weare looking for an individual who has the confidence to challenge decisions atall levels, experience of working in a fast paced and high-pressureenvironment, and the ability to recognise the need to escalate complexdischarges to senior management to prevent discharge complexities.We have a reputation forproviding a caring and supportive style working environment for our staff andyou will find that we are determined to work towards improving health,preventing unnecessary admissions to hospital, and facilitating earlydischarges by caring for patients.
Main duties of the job This role will work as part ofthe Early assessment team, through a multi-disciplinary team approachworking alongside colleagues from Therapy services, Social services and the Acutetrust.
Promoting positive staffbehaviour, and facilitating the delivery of treatment to individuals, groups,and communities.To work collaboratively tosupport the delivery and development of integrated care across the WakefieldDistrict, working within the acute trust.The successful applicant will have good communication skills, IT skills, and a flexible attitude to working patterns and environments.
Your work pattern will include shifts and weekends.

Job description Job responsibilities To supervise and lead staff within the Integrated Care Team in the absence of the Team Leader promoting positive staff behaviour and facilitating the delivery of treatment to individuals, groups and communities.
To take full responsibility of the service caseload in the absence of the Team Leader.
To analyse and triage referrals received by the service against the eligibility criteria and using own clinical expertise.
Taking appropriate action to ensure the health needs of patients are met.
To provisionally identify with the referrer what the goals and aims of the interventions will be.
To adopt a problem solving approach for referrals considered not appropriate for the service, sign posting and redirecting to other services as appropriate.
To discuss contentious referrals with the Team Leader or Locality Matron in their absence.
To act in a professional manner at all times whilst on Mid Yorks business and demonstrate excellence in clinical practice.
To demonstrate and deliver high levels of holistic assessment within the patients own environment, utilising appropriate assessment tools, contribute to the development of patient centred care plans and to make referrals as appropriate to other agencies.
To understand and have awareness of the importance of collaborative partnerships with other multi-disciplinary agencies.
To assess and plan individual patient care plans, education and treatment reviews.
To effectively discharge patients from the service and ensure patients are sign posted and/or referred to other services either statutory or voluntary should ongoing health or social needs be identified during discharge planning.
To perform risk assessments ensuring a safe environment within patients homes and work areas.
Ensures adherence to organisational Health and Safety, risk and governance arrangements.
To demonstrate skills in communicating with patients and relatives/carers who have barriers to understanding such as language, sensory impairment, confusion and aggression ensuring ethnic considerations take place.
To actively be involved in the promotion of healthy life styles, secondary prevention, health promotion and patient education.
To work autonomously to competently administer intravenous therapy, cannulation and phlebotomy.
To maintain own knowledge base and experience that reflects the current evidence based practice in relation to IV Therapy.
To lead in the continued development and provision of a Community Integrated Care Service which will impact to reduce hospital admissions, facilitate discharges to reduce in-patient hospital bed days and reduce the incidences of healthcare associated infection.
To provide accurate statistical information as required.
Review medications, seeking advice from local pharmacists and the drugs information hotline as appropriate.
If legally authorised, prescribe medications from agreed formularies based on efficacy, safety and cost.To maintain confidentiality, privacy and dignity in relation to the patients and their treatment at all times.
To delegate work to nurses and health care support workers accordingly to ensure appropriate and safe use of skill mix.
Demonstrate the appropriate supervision skills with mentorship, achievement of learning competencies and ongoing training and development.
Take responsibility for the delegated Appraisal and achievement of learning competencies for a designated number of health care support workers.
Participate in clinical supervision ensuring that practice is maintained at high levels and evaluated utilising appropriate mechanisms.To lead in the development and delivery of educational and competency based assessment of clinical staff including intravenous cannulation and the administration of intravenous therapy through any identified vascular access device.
To deliver educational opportunities for nurses and health care support workers.
To act as a mentor for pre-registration nursing students.
To maintain standards in ongoing health information, both electronic and paper based, ensuring safe keeping and confidentiality, accurate record keeping of relevant data and profiling.
To input clinical information onto SystmOne as per Mid Yorks policy.To adhere to Mid Yorks policies, procedures and protocols to promote the highest quality of care within the patient experience.
Person Specification Experience Essential Understanding the implications of early discharge and admission avoidance, considering interdisciplinary needs and implications for patients needing intermediate care.
Community nursing/primary care experience Teaching and educating skills facilitating the learning of others Experience and Understanding of integration.
Experience of multi-disciplinary team working.
Experience of change processes Extended clinical skills to include experience of clinical supervision/reflective practice and experience of clinical audit Desirable Palliative Care experience / qualification Competency in IV cannulation and IV Therapy Administration.
Knowledge and Awareness Essential Knowledge of relevant government policy and the modernisation agenda.
Awareness of own limitations.
