*Direct Homecare *is now seeking a Nurse that we can train to work with us in the field as a Clinical Lead in the London area.

*Role Overview:* As a Clinical Lead, you will play a pivotal role in delivering high-quality care to our patients.
You will be responsible for conducting assessments, developing care plans, and providing ongoing support to ensure the best outcomes.
Your role will involve visiting patients in their homes, within the community, or in hospital settings to perform thorough assessments and coordinate with the home care team on the required care and support.
*Key Responsibilities:* - Conduct comprehensive assessments of patients' needs.
- Develop and implement detailed care plans tailored to individual requirements.
- Provide ongoing support and supervision within care packages.
- Collaborate with the home care team to ensure seamless delivery of care.
- Visit patients' homes, community settings, or hospitals to assess needs and discharge readiness.
- Participate in ongoing reviews and provide support for staff within care packages.
- Complete multi-sign-offs as required, with appropriate compensation for each task.
*Requirements:* - Qualified Nurse with an active NMC Pin .- Strong assessment and care planning skills.
- Excellent communication and collaboration abilities.
- Ability to work independently and manage time effectively.
- Flexibility to travel to various locations as required.
You will receive comprehensive training to meet the expectations and requirements of the Clinical Lead role.
Ongoing support will be provided by a Clinical Manager and Clinical Director.
Competitive pay rates, including additional compensation for multi-sign-off tasks, will be offered based on the work performed.
