Job summary Nurse Associate - Band 4Devizes Community Team Hours per week - PermanentWiltshireHealth and Care is looking for an enthusiastic and highly motivated NursingAssociate to join Devizes Community Team in Wiltshire.Wework with a wide range of partner organisations to deliver care and supportpeople in their own homes and from a number of hospitals and community bases.We focus on delivering care as close to home as possible.Inthis role, you would be working as part of a Community team incorporating:Community Nursing, Clinical Leads, Physiotherapists, Occupational Therapists,Healthcare support workers, Rehab support workers and Care Coordinators.
Youwill link closely with colleagues including general practitioners and othermembers of the multidisciplinary team to support excellent nursing care athome, reducing the necessity for acute hospital admission.
Main duties of the job Thisrole is aligned specifically to the community nursing team providing nursingsupport to patients in their own homes.

Person Specification Qualifications Essential Level 5 Nursing Associate qualification NMC registration and Nursing Associate Desirable UK Core Skills Framework Mandatory training Experience Essential Some experience of working within a health and social care environment Ability to communicate with members of the public and health and care providers Courteous, respectful and helpful at all times Ability to deal with non-routine and unpredictable nature of the workload and individual patient contact Knowledge of when to seek advice and refer to a registered care professional Desirable Ability to work on own initiative Ability to take part in reflective practice and clinical supervision activities Understanding of evidence based practice Understanding of basic physiology, normal vital signs, fluid balance, nutritional requirements etc.
