Job summary Fixed-Term Contract - 12 months We have an exciting opportunity to recruit an enthusiastic Band 4 Nurse Associate on a 12 month fixed term contract, to join Peterborough Adult Locality duty team.
You will be working alongside a Band 7 duty lead supporting patients who are experiencing a crisis, administering depots, planning and delivering care to patients on the care coordination waiting list.
You will have daily Multi-Disciplinary Team support.
The role requires the individual to work from base but be flexible to travel within Peterborough community at short notice.
Main duties of the job The post holder will provide quality evidence-based nursing advice, support and care for service users with mental and physical health needs and to their carers within a multidisciplinary setting.
They will be responsible for the assessment, planning, implementation and evaluation of nursing care in line with the agreed NMC standards specific to the role, and will have a working knowledge of local, national policies and legislation which govern current service provision.
The post holder will ensure that effective communication pathways exist, and that they act within their sphere of responsibility.
They will work as part of the duty team to manage the unplanned work which comes into the duty inbox.

For further information on CPFT, please visit our website at Job description Job responsibilities Please refer to the attached job description and person specification for full details of responsibilities To be responsible for the assessment, planning, implementation and evaluation of the evidence-based care required, including health promotion for a named service user/group ensuring it meets Trust timescales in line with agreed NMC standards.
To support the Clinical lead and other staff on the duty rota to manage, where appropriate, unplanned work.
To participate in the setting of quality standards, including the monitoring and reviewing in line with current clinical guidance practice and policy.
To support the Clinical lead, where appropriate, with practical interventions to de-escalate crisis situations.
To ensure the post holder understands and meets their professional role and responsibilities under the Care Programme Approach To support the duty clinicians to manage the duty inbox effectively to ensure that requests for help are dealt with appropriately and in a timely manner.
To work from the team base alongside the rest of the duty team to provide immediate support to team members and admin staff dealing with unplanned work.
Where appropriate help to identify early signs of relapse by monitoring the users progress, level of functioning and mental state and alert the appropriate staff involved in the individuals care.
Develop good working relationships and work collaboratively with other organizations that are able to help individuals resolve crises, including vocational and educational providers, training providers, voluntary agencies, accommodation and financial services.
To work using assertive outreach approaches and manage your diary appropriately.
To support the Clinical lead to build more effective pathways with the acute services.
To ensure that the requirements of the Mental Health Act 2007 are observed, adhered to and implemented.
To promote carer and service user involvement within the service and for its provision.
Person Specification Education / Qualifications Essential oRegistered Nursing Associate on the NMC register.
oEducated to a minimum Level 5 Foundation Degree specific to Nursing Associate Practice.
oGCSE's Grade A - C in Maths and English or equivalent Desirable oQCF in Care / Health & Social Care Level 2/3 or equivalent experience oCompleted Care Certificate Experience Essential oExperience of working as part of a multi-disciplinary team oBasic level of dealing with service users with complex health needs oPrevious work with people suffering with serious mental distress Desirable oExperience relating to working with serious mental illness.
oLived experience of mental health oExperience of having given depot medications.
Skills & Abilities Essential oExcellent interpersonal and communication skills oIT skills appropriate to the role oAble to recognise own limitations in the context of the nursing associate role oAbility to work well in a team.
oAbility to work on own initiative oAbility to organise and prioritise own workload.
oKnowledge of when to seek advice and refer to a registered health or care professional oAbility to participate in reflective practice and clinical supervision activities.
