Job summary

YICT support patients toremain independent in their own homes, while continuing with healthcareassessments, rehabilitation, and recovery.
We also provide an urgent crisisresponse supporting patients and preventing unnecessary hospital admissions.
You will work closely with ateam of healthcare professionals to support patients' individual needs andpromoting patient health and wellbeing.
Please note: this role includes evening and weekend working.
Main duties of the job
To provide first visit assessments of patientsin their own home, and ongoing care andsupport to the individual.
Assist patients to meet their basic needs,including prompting and administering medication as required, supporting withdietary needs, maintaining personal hygiene, and using toilet facilities asappropriate.
Perform physical observations on patients usingthe necessary equipment (blood pressure monitors, blood sugar, and pulseoximeters) when appropriate or when advised to by a manager or member of thenursing team.
Ensure patients comfort, safety, and dignityat all times, respecting privacy and ensuring confidentiality at all times.
Assist and support patients and carers as appropriate.
Ensure referrals to other agencies are made andeffectively and efficiently signpost patients to relevant agencies whenappropriate.
Ensure relevant information, including allchanges in a patient's condition, is passed to the registered nurse on duty.Ensure efficient hand over of visit allocations with Healthcare Supportcolleagues.
Ensure accurate records are maintained in atimely manner including discharging and archiving patient paperwork, completingHealthcare Support visit data and CQC documentation.

With excellent communicationskills you will have the ability to build a professional rapport withpatients', being able to calmly and professional identify and deliver patientneeds.
A team player, you will workeffectively and professional within the wider YICT team and also have theability to work independently,
You will have a can-doattitude and be able to react to challenges or setbacks in a positive manner.You will be able to work efficiently.
You will hold a full UKdriving licence and have access to a vehicle.
Full training will be given,we invite applicants with no experience in healthcare.
