The Growth Company (GC) has an exciting opportunity for an NHS Integration Coordinator to join us on the IPSPC contract throughout Manchester.

This is an engaging opportunity that requires experience in a health-based partnership role and strong influencing and relationship development skills.
The successful candidates will be integral to working with key stakeholders and partners to secure co-location within primary care services and generate referrals to the programme.
Key Responsibilities * Responsibility for brokering and establishing relationships with the wider health services that interact with our participants, including primary care teams, NHS and other health care services, mental health teams and Voluntary and Community Sector (VCS) providers to generate referrals to the programme.
* Work with key stakeholders to establish co-location which will benefit the participants and provide greater levels of cohesion between services.
* Maintain and develop relationships with targeted stakeholders, partners and community groups to drive referrals to the programme and drive engagement.
* Deliver presentations, workshops and other activities within stakeholder, partner and community groups to generate referrals for key contracts.
* Undertake project work to support the generation of referrals, including planning and delivery of events and activities to access potential participant groups.
* Record daily engagement activity on the relevant systems to provide accurate Management Information.
About you * You must have previous knowledge of the primary health care sector, the employability sector, nationally and in the area of delivery.
* Knowledge of key stakeholders and partners in the area of delivery.
* Previous knowledge of the community & voluntary sector is essential.
* Targets and results-driven, balanced with a real people focus.
Skills Required * Experience in integration and partnership work within the primary healthcare sector.
* The ability to build relationships with key stakeholders and partner organisations.
* Previous experience and knowledge of the primary healthcare sector is essential.
* Excellent and highly developed communication skills, capable of interacting with different-sized groups of participants with various barriers and needs.
* Strong IT skills with experience in using databases, websites, and social media.
* Able to demonstrate partner engagement at a high level.
* Project management skills including organising and planning events.
