Job summary Accuratelyexamine and assess patients with an undifferentiated diagnosis, from aphysiological, psychological and social perspective, and plan clinical careaccordingly.Examineundifferentiated and undiagnosed problems, making use of skills in historytaking, physical examination, problem-solving and clinical decision-making todiagnose and manage both acute and chronic conditions, integrating both drug-and non-drug-based treatment methods into a management plan.
Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergency medication in accordance with PGD medication list as approved by the supervising information and advice on prescribed or non-prescription medication, onmedication regimens, side-effects and interactions Ensurefull and accurate documentation in patient notes of clinical history, physicalexamination, diagnosis and treatment clearly communicate the health status of the patient using appropriate terminology,technology and format and maintain respect and dignity for the patient at alltimes.
Towork within the practice guidelines, policies and protocols.
Tomaintain accurate and contemporaneous records, utilising computer systems whereappropriate and consider the Caldecott Principles in relation to all datahandling.
Main duties of the job The post holder will be an experienced clinician, who acting within their professional boundaries; will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their appropriate clinical assistance, they will demonstrate safe, clinical decision-making and expert care for patients within the general post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and post holder will build effective working relationships with other health care post holder will work within the clinical on-the-day team to deliver excellent primary health care services within the practice.

Qualifications Essential You'll need a minimum of five 9-4 (A*-C) grade GCSEs (or the equivalent) Desirable Have an approved degree in paramedic science/practice and be registered with the Health and Care Professions Council (HCPC).
