Ideally for 4-5 sessions per week.
This would be on an initial temporary basis, but with a viewto a permanent role once NHS England have confirmed funding for these ARRSroles for 2025/26.The role may require you to work within our Enhanced Accessset up on evenings and weekends and therefore a level of flexibility isrequired.
Main duties of the job All general duties and responsibilities associated with a GPworking within primary care:Undertaking a variety of duties includingsurgery face to face consultations, telephone consultations and queries,visiting patients at home, prescription administration and management, general administration ( referrals, pathology, reports,correspondence ) in a timely fashion, ensuring high quality care.Making professional, autonomous decisions inrelation to presenting medical problems, whether self-referred or referred fromother healthcare workers within the organisation.Assessing the healthcare needs of patients withundifferentiated and undiagnosed problems.Screening patients for disease risk factors andearly signs of illness.Developing care plans for health in consultationwith patients and in line with current practice disease management protocols.Providing health education.Contribute to optimise PCN Investment and ImpactFund ( quality improvement markers)Recording clear consultation notes to agreedstandards.Collecting data for audit purposes and complyingwith QOF requirements.Delivering enhanced services.Compiling and issuing computer-generated acuteand repeat prescriptions.Prescribing according to local and nationalguidelinesSupporting Extended Hours services, enhancingpatient access About us The Five Lane EndsPCNs consist of four practices in total covering a total population of over 35,000patients.
Five Lane Ends PCN consists of 4 GP practices Moorside surgery,Ashcroft surgery, Farrow Medical Centre & The Rockwell & Wrosepractice.

This shared approach is allowing us to rapidly build on the pro-activecare model we have been developing as part of the Care-Coordination Act asOne programme in Bradford District and Craven.
We have already beenrecognised for our innovative integrated approach and celebrated at the Act asOne festival.
We are also currently sharing case studies with NHSE and the HSJ asan example of good practice in utilising the PCN DES contract innovatively.
