Job summary PENNINE HOME TREATMENT TEAMBased Daisy field Mill Blackburn and Pendle House NelsonMental Health Practitioner (Band 6) hrs per week, including nights and unsocial hours as per rota requirements.Pennine Home Treatment Teams are split into 2 locality bases covering the Pennine Area.
We have an exciting opportunity for forward thinking, highly motivated, innovative, dedicated practitioners to provide care and treatment for those service users who require support from the Home Based Treatment Team.Practitioners in the team are dedicated to working with vulnerable service users in their own homes, preventing further deterioration to avoid the need for admission to hospital.
We have close working relationships with Community Mental Health Teams and other services across the network such as Mental Health Liaison, Frequent Attenders and Early Intervention teams.
We work closely with the Initial Response services where you will also be expected to be located at times to support the pathway.You will be able to demonstrate that you reflect Lancashire Care & South Cumbria NHS Foundation Trust (LCSFT) values of Teamwork, Integrity, Compassion, Excellence, Respect and Accountability.

The bonus is only available for people who are not currently employed (or who have not been employed in the last 12 months) by any NHS provider trust in the Lancashire & South Cumbria Integrated Care System in a like-for-like role; a Band 6 Registered Mental Health Nurse at another Trust in the LSC ICS would not be eligible for the bonus if moving to a Band 6 Registered Mental Health Nurse role at LSCFT.
Bank only workers for Lancashire and South Cumbria NHS Foundation Trust are eligible for the scheme if they have not held a substantive role in Lancashire & South Cumbria Integrated Care System in the last 12 months (subject to all other terms and conditions being met).
These incentives are subject to recovery clauses within the first 6 months of employment.
