"Closer2Home is a specialist intensive home treatment service that supports young people with acute mental health problems in their homes by providing intensive input and support using a bespoke package of care tailored to each young persons needs.We operate 365 days a year from 8am-8pm to offer flexibility to young people at times when they need it the most.
The teams work in an integrated way, supporting one another to provide the service either remotely or sometimes by crossing borders into another trusts area.
This means that a YP's needs are not restricted by boundaries, staff sickness has less of an impact on service delivery and YP receive a better service where ever they live.

Hospital admission can be lifesaving but often has a detrimental effect on young peoples as well.
Closer2Home provides a service that helps young people in crisis to remain in their own home, in their own community where they can get involved in meaningful activities that are worthwhile to them.
Main duties of the job Closer2Home:Provides assessment and intensive support for young people with mental health needs who present high levels of riskSupports assessment prior to admission to inpatient care (hospital), where requiredDelivers planned intensive home or community-based treatment services, as well as specialist intensive home treatment for young people whose needs exceed the threshold for Tier 3 CAMHSSupports discharge to facilitate timely discharge from hospitalSupports the reduction of health inequalities in children and young people mental health pathwaysEach team has been modelled to have an optimal number and skill mix of staff to effectively address the needs of the young people the service will support.In the role, you will:Work collaboratively with community CAMHS, children's services, colleagues involved in 'transforming care,' bed and case managers and local inpatient units; using an interagency approach to provide the best care for the young people.Be expected to provide clinical leadership to their local Closer2Home teamBe integrated in a Wessex & Dorset wide system of clinical and operational governance, a new infrastructure that will utilise robust pre-admission care-planning, a 'single point of access' for intensive home treatment and inpatient services, goal setting at admission and supported inpatient discharge.

Person Specification Qualifications Essential Nationally recognised core qualification RN, RMN, Social work, Psychology Professional Qualification at first degree level or equivalent Completion of further post-qualification specialist training at post graduate level Experience Essential Experience of specialist mental health assessment and treatment of children and adolescents and their families/carers where appropriate as a full member of a multi-disciplinary team located in community, primary care, outpatients or inpatient settings Experience of working with a wide variety of children, adolescents and their families/carers across the whole range and presenting problems that reflect the full range of clinical severity.
Post qualifying experience of working as a specialist mental health worker within a mental health setting
