Mental Health Practitioner

Job Title: Mental Health Practitioner (MHP) – CAMHS Case Manager Tripod Partners are currently recruiting for a Band 6 Mental Health Practitioner (MHP), RMN or Social Worker, to join the West Lancs CAMHS team in Skelmersdale.
This is a full-time role, Monday to Friday, supporting young people with mental health needs.
Key Responsibilities Serve as a Case Manager, providing support and care coordination for children and adolescents within the CAMHS team.
Conduct initial assessments (IA) and participate in ward and duty duties as required.
Work as part of the multi-disciplinary team (MDT) to develop and review care plans and interventions.
Participate in team meetings to ensure cohesive and comprehensive care for all patients.
