Mental Health Nurse

Full time Monday to Friday 9-5, Can have some flexibility in hours, and may be asked to work an occasional additional day To complete the ongoing assessments for young people at crisis on the ward, or routine and in the community.
* To complete all assessment paperwork, contact any agencies and make appropriate referrals.
* To complete risk assessments as appropriate and safety and care plans * To work with other partner agencies eg children's services / safeguarding etc.
* To look at undertaking ongoing support work for young people in crisis, to refer as appropriate and then review on return to services.
* To complete sessional work with the young people on an ongoing basis as work directs.
* To work with risk and complexity and priorities * To manage a caseload and escalate any matters of concern or uncertainty.
* To work in a team and support and be supported by the team.
