The pay grade on this basis would be Band 7 Annex 21 - £36,966year one, then £39,606 year twoThis is anexciting opportunity with South Yorkshire Integrated Care Board to join ourprogressive and multi award winning Medicines Optimisation Team as a Pharmacist.You will have the opportunity to help shape the medical care of the populationof SY, working to remove inequalities in accessing services and improve healthoutcomes for patients in the area.

Main duties of the job Have knowledgeand understanding of the ICB Quality, Innovation, Productivity, Prevention(QIPP) plan and support its implementation at GP practice level.Have knowledgeof guidelines and policies developed in response to NICE or NHS Englandpublications.Assist ininformation sharing relating to medicines, including national and localprescribing policies, prescribing committee decisions, and medicines safetyissues.Assist withinvestigation of medicines related safety incidents, CQC concerns and supportthe development of systems and processes.Support theprovision of appropriate medicines optimisation education and training eventsfor healthcare professionals.Buildrelationships and influence GP practice and PCN teams to ensure effectivecommunication on issues relating to prescribing and population health needs.Provide GPpractices and PCN teams with timely and accurate prescribing performance data,supporting them to manage their prescribing budgets whilst adhering to localguidelines and formularies.

We workalongside all these colleagues through local councils, our VCSEpartners and other partners to address health inequalities and widerdeterminants of health in South Yorkshire.South Yorkshire ICB is a Disability Confident Employer Job description Job responsibilities To provide and receivecomplex or sensitive information to patients/carers, GP practice staff, othermembers of the Medicines Optimisation Team and wider ICB and ICS.To communicate results ofaudits and other prescribing related work to ICS partner staff or concisely,presenting information in a suitable format ( written and/or verbal) andstyle for the target audience.To attend and contribute to partner provider meetingsand other prescribing meetings, on issues relevant to prescribing and medicines optimisation.To assist with themonitoring and analysis of complex prescribing data and offer support topartners who are outlying on key prescribing indicators.
To undertake audits of SYICS partners to assess prescribing against local and national guidelines andprovide an interpretation of the outputs to colleagues.To interpret and makejudgements using clinical and non-clinical data, which may be incomplete frompatients notes and other sources, alongside organisational and nationalguidance to facilitate patient care and specific issues, ICB priority areas andlocal /national guidelines.
To consider a range of potential options and beconfident in deploying one of those options.To plan and organise ofmultiple tasks, activities, or programmes some of which more be complex andconcurrent to be delivered to ICS partners or colleagues training on aparticular medicines topic, medicine changes.
Planning own work to meetassigned targets, some of which may be ongoing work.
Co-ordinate with pharmacy technicians andproject/ program leads, as well as the service requirement of the ICS partnerreceiving support.To provide, if deemednecessary by the ICB, direct patient care and consultation related to healthinitiatives.To provide specialistclinical advice to practitioners and patients based on interpretation ofmedical data to improve the care of an individual, or groups ofpatients/clients in the care settings supported by the ICB.To work with ICS partnerstowards the delivery of the medicines optimisation Quality, Innovation,Productivity and Prevention (QIPP agenda) and ICS IPMO agendas, implement andsupport local formularies, and other prescribing objectives and strategies as determinedby the strategic team.
Feedback to line managers and colleagues whereimprovements could be made across the spectrum of work encountered.Postholder should observetheir personal duty of care in relation to IT equipment providers and ensureprotection of data held in line with the information sharing protocols.To support with thetraining and induction of new and existing staff members.To deliver agreed coretraining to provider colleague staff members, and ICB staff on specialist areasof involvement.To commit to continualprofessional development as per GPhC requirements and maintain a personalportfolio.To accurately make agreedchanges to patients medicines and update patients records with details of thechanges made and to update records in a timely manner at the time the change ismade.To work with members ofthe team to develop and implement data collection systems that will provideaccurate and timely data.To develop and maintaindatabases required by job.To maintainadministrative and information resourcesTo advise and demonstrate own activities toother colleagues.To participate inresearch to ensure pharmacy services, medicines optimisation and clinicalresearch involving medicines are provided, developed, and undertaken inaccordance with relevant governing frameworks and ethical and qualitystandards.To evaluate and present the results of the reviews andaudits conducted in providerpartners, bothindividually and collectively, in conjunction with other members of theMedicines Optimisation Team working in the practice.
To work on own initiativeand unsupervised to design and manage workload to meet set objectives, guidedby ICB and NHS processes, peers, and experience.
To deliver against agreedobjectives, achieving quality outcomes assigned by line managers.
To organise meetings orevents in connection with portfolio of work.
Light physical effortrequired, carrying of laptops, projectors, and light event equipment.
There is a dailyrequirement for working at a workstation for much of the day.
Frequent requirements forprolonged concentration for much of the working day, , checking documents,analysing statistics, and conflicting priorities.
Elements of the work will beunpredictable with need to switch task without warning to support a situationof event stock shortage, procurement of service, staff cover of adifferent area of work within Medicines Optimisation.
Expose to accountsinvolving patient that may cause distress, occasional encounters with patientsor practitioners that involve the imparting of difficult or unwelcomeinformation.The role will be a hybridworking environment, between home and base location utilising IT on daily basisfor most of the time, but also travelling to other Acute Trusts / placelocalities across the South Yorkshire locality.
Remote working is often part ofa hybrid approach.
Postholder must be able to independently travel around thelocality.
