Job summary We have an exciting opportunity for a Pharmacy Technician to join our Medicines Management Team based across Burnham Medical Centre and Highbridge Medical Centre.
You will work alongside a team of pharmacists, pharmacy technicians, prescription clerks.
You will assist with medicines reconciliation, support drug monitoring work, and answering medication queries from patients and community pharmacies, as well as supporting the practice with all aspects of medication safety.
You will liaise with GPs, practice staff, community pharmacies and secondary care colleagues, and have direct contact with patients and carers.
You will need to demonstrate the ability to use your initiative, have a positive attitude, a can-do approach towards problem solving, and be a good team player.
Full training will be given.

Job description Job responsibilities JOB SUMMARYThe post-holder will support and assist activities in all of the surgeries within the South Somerset West Primary Care Network to help patients to get the best from their medicines by supporting systems to improve the continuity of care, handling of prescription queries from patients and health care professionals and supporting medication management in care homes.

To support the prescription clerk team, including: Liaise with patients and pharmacies regarding prescription queries and requests.
Work with the practice medicines management teams, clinicians, and other practice staff to resolve all medicines related queries.
Liaise with hospital staff with reference to discharge and clinic letters.
Check patient records for chronic disease and drug monitoring and arrange for testing and review as appropriate.
Enter relevant patient information accurately onto EMIS.
Use the Prescription Tracker to establish the status of electronic prescriptions.
Undertake any other duties appropriate to the post as requested.
