Role Summary To provide a safe and cost-effective Medicines Management Pharmacy service to the AHAH service.
To be a source of medicines advice to colleagues, patients and key stakeholders with respect to medicines optimisation for AHAH patients.
Facilitate safe transfer of patients to and from AHAH service (including discharge) with respect to medications by ensuring appropriate medicines are available and optimised.
Communicate effectively and promptly with stakeholders to ensure relevant information available to Health and Social Care staff, patients and family members following patient transfer and discharge with respect to medicines To liaise with prescribers and members of the multidisciplinary team to resolve discrepancies with patient medicines to facilitate care.
To support other services within the Pharmacy department including Dispensary service.

More detail about the role This post provides a unique opportunity to join the pharmacy team at the Royal Devon University Healthcare NHS Foundation Trust and enhance our ability to provide medicines support for Acute Hospital at Home patients (also known as Virtual Ward).
The successful candidate will be based in Barnstaple or Exeter and work to support patient care for AHAH patients across our Northern and Eastern services.
Patients receiving treatment from the Acute Hospital at Home service receive the same care, monitoring and treatment as other inpatients in an acute hospital bed but from their own home.
You will work as part of the multidisciplinary AHAH team which includes a senior clinical pharmacist as well as Doctors, Nurses, Therapists and Clinical Support Workers.
Further information about the AHAH service within Royal Devon is available on our Trust website here: We are looking for an innovative and experienced Medicines Management Pharmacy Technician to join our team and enhance our ability to provide medicines support for Acute Hospital at Home (Virtual Ward) patients.
This is an exciting, new, substantive role that will allow you to develop working relationships with members of the multidisciplinary team and Pharmacy colleagues within the hospital and community setting as well as linking with colleagues in Primary Care, Social Care and Community Pharmacy.
The role is ideal for an experienced Pharmacy Technician who has sound knowledge and experience of hospital pharmacy at ward level supporting medicines reconciliation, has a good understanding of the discharge process and a passion to improve the quality of care for patients within our AHAH service.
Having a wider knowledge of Pharmacy systems and processes within Primary Care, Community Pharmacy and Social Care setting would be an advantage for this role but are not an essential requirement.
The successful post holder will possess excellent communication skills to support resolution of medicines management issues and understand that the majority of patient interactions will not be face to face.
Working knowledge of the Discharge Medicines Service (DMS) to Community Pharmacy would be an advantage.
You should be a highly self-motivated individual with an ability to plan and work flexibly, independently and as part of a wider team.
The AHAH service is a fast pace environment providing care for patients on a variety of clinical pathways.
Candidates should note that this is a joint post supporting AHAH patients within Northern and Eastern services and therefore an ability to travel to other locations periodically is a requirement of the role.
