Job summary Having strong communication and people skills is fundamental as you will be required to work in partnership with the multi-professional team, ensuring every patient and their needs are always at the centre of what you do.
You will draw on your expertise to carry out a holistic assessment of patients and work collaboratively with the other healthcare professionals to provide excellent care for our patients and their families.

Main duties of the job This post will be a key part of the Consultant team.
You will primarily support our In-Patient Unit, looking after hospice patients with a Multi Professional team approach, providing urgent and planned medical reviews for patients as needed.
You will also provide regular support to the Wellbeing Centre, as well as cover for our Hospice at home.
You will be available to support and provide cross-cover to consultant medical team colleagues.

Job description Job responsibilities To liaise closely with, and support, consultant colleagues.To work closely with, and support, all members of the widerMulti Professional Team (MPT).Toparticipate in the weekly in-patient ward round and provide medical leadershipwhen needed.Toprovide ward reviews and Consultant ward-cover when needed and provide clinicalsupervision and support to middle grade staff.Towork closely with external healthcare professionals GPs, district nurses andhospital specialists to provide support, advice and continuity of care.To manage hospice patients within an MPT approach, providing urgent and planned medical reviews for patients when in their home, as an outpatient or as hospice inpatients as needed.To complete holistic, medical assessments and formulateclear management plans, in close collaboration with the rest of the MPT, patients and theirfamilies.To maintain ongoing medicalcare for Princess Alice Hospice (PAH) patients, reviewing signs and symptoms and to negotiate / update management plans accordingly with therest of the MPT, the patients and their families.To support / address the needsof carers / families, communicating with them as often as necessary.To maintain accurate and clear,contemporaneous clinical records appropriately updated electronic patient records.To provide excellent communication / handover of clinical informationby telephone, email or in writing as appropriate, ensuring that all relevant in-house / externalhealthcare professionals are up to date with clinical events letters to general practitioners.To ensure continuity of care.To attend appropriate community MPT meetings.To coordinate with colleagues to help in the prioritising of admissions to and discharges from; ESS; the communitycaseload; and the inpatient unitTo coordinate with colleagues to assess and manage patients in the Wellbeing Centre as necessary.
Also, where appropriate, discuss with their Clinical Nurse Specialist (CNS) and / or GP in a timely fashion.To work flexibly with themedical team and to provide cover for colleagues and the wider MPT as appropriate.To help with induction,education and supervision of medical students and visiting doctors as appropriate.In line with national changes,plans for 7-day working have tobe considered, future work patterns, including on-call patterns, may need to changein line with this.General requirements for medical staffTo participate in audit, research or any on-goingdevelopments within the hospice and, where appropriate, to develop their own audit / research.To participate in the medicaldepartments Continuing Professional Development (CPD) programme Monday meetings and the Wednesday / Thursday medical journal club (alternates monthly), as well as other educational activities at the Hospice.To participate in teaching forthe wider MPT, hospice staff / volunteers and externally where appropriate.To comply with CPD requirements as stipulated by the Royal Colleges.To keep up to datewith clinical mandatory trainingrequirements.To keep a portfolio ofevidence, to have an annual appraisal and write a personal development plan(PDP) and objectives in line with GMC revalidation policies and procedures.To maintain their ownprofessional indemnity insurance to cover all work in the hospice.To maintain confidentiality atall times and comply with all other aspectsof the General Medical Councils Code of Professional Conduct / Good Medical Practice.General requirements for all Staff:To attend the inductionprogramme.To comply with OccupationalHealth procedures.To attend any staff meetings,where appropriate, in discussionwith the Medical Director.To attend statutory training, fire training, moving and handling andother health and safety issues.To act at all timesin a manner to safeguard the interests of patients andtheir families.To be aware of and performaccording to the Hospices values and behaviours.To monitor, record and assist in investigationsof any complaints from patients, visitors and staff, reporting these to the Medical Director.To be fully conversant with, and participate in, the hospices Governance Policies.
