The Community Crisis Response Team is based atMount Gould Hospital and is a team of approximately 60 staff members of varyingprofessions The vacancy is a permanent contractfor a Community Support Worker who support clients in the community and goneinto crisis and to help avoid hospital admission Shifts of 3 x 10.5 hours between 8.00am and 8.00pm Towork as a generic support worker undertaking specific tasks to support clientsreceiving intermediate care.
The postholder will be required to work alone with clients in various settings in thecommunity without direct supervision Youwill need to be able to drive between clients during your shift Tomanage your own work priorities, balancing clinical, professional andorganisational demands Totake a flexible approach to supporting colleagues in times of caseload pressure Tomaintain accurate, comprehensive and up-to-date client records and relateddocumentation in line with clinical guidance and LWSW policy Tocomply with LWSW policies Totake reasonable care for personal health and safety Toundertake any other duties considered reasonable and appropriate by CCRTmanager All Livewell Southweststaff are expected to be able and willing to work across a 7 day service.
