We are seeking dedicated postholders who willwork collaboratively with the general practice team to meet the needs of our patientsthrough the delivery of safe patient care.
This role hasan essential criterion of holding a level 3 Healthcare Support Worker qualification and is not a training role.The post holderwill support as part of the wider nursing team across Modality Hull MedicalPractices and provide weekly visits to care homes to promote the health and wellbeing of our patients by assessing theirneeds, providing pre-screening activities and health care for chronic diseases,health education advice and support, implementing requirements in line withpatient requirements, community, or other settings, maintaining clinicallyaccurate records during clinics.This job issuitable for individuals who can demonstrate safe and effective patient interactionsensuring the delivery of high-quality patient care whilst working alongside thewider nursing team and practice team.

The postholderwill be a key member of the practice team working under the line management ofregistered Practice Nurses.
The postholder will be responsible for undertakingpatient health checks to include observations such as BP monitoring, ECGmonitoring, height and weight, phlebotomy, and provide health and wellbeing adviceand promotion such as smoking cessation advice and signposting to requirementsin line with the patients health needs.

Person Specification Qualifications Essential GCSE Grade A-C Maths and English (Or equivalent) Level 3 Healthcare Support Worker qualification Skills Essential Recent previous experience in direct patient care.
Phlebotomy skills Proficient in Blood pressure monitoring Urine testing Excellent communication skills Ability to follow guidelines and protocols.
