Job summary The position is within the older persons with physical disability team which is a community-based team working with Adults over 18 years of age with a range of physical and Mental health is a dynamic post that requires the post holder to work closely with multi-disciplinary teams/agencies in addressing a range of care/support issues in the promotion of independence and improving the health and wellbeing of the population served.
As a level 2 social worker, you will hold at least 2 years post qualifying experience.
You will have responsibility for your own case load and will work on a range of cases involving assessment, support planning, mental capacity Act assessments, appointeeships and deputyship referrals.
Section 117 reviews and safeguarding enquiries as well as complex case management.
There is opportunity to manage your own workload and diary and you will be offered the opportunity to work in a hybrid way, being based between the office base in Stafford and home working.
Main duties of the job The post holder will be expected to access Social care and health records to gather information, therefore it is important for the post holder to be able to use a computer and have an understanding of the importance of good recording.
You will require effective communication and written skills and be willing to work as part of a team to promote best outcomes for individuals.

Social Work Learning Academy (SWLA) On appointment, the post holder will be auto-enrolled in the SWLA and aligned to the appropriate British Association of Social Work Professional Capabilities Framework.
