Job summary The post holder will support the Southwest London ICS Learning from Lives and Deaths - People with a learning disability and autistic people Programme ( LeDeR ) with the completion of LeDeR reviews.
LeDeR is the National service improvement programme to improve care, reduce health inequalities and prevent premature mortality of people with learning disability and autistic people by reviewing information about their health and social care.
It is commissioned by NHS England and is part of the NHS Long Term Plan.
The LeDeR programme has been running since September 2017, co-ordinated and delivered by SW London ICB with the support of a wide range of partner organisations.
A key part of the LeDeR Programme is to support local areas to review the deaths of people with learning disabilities.
The purpose of the reviews is to collate and share the anonymised information about the deaths of people with learning disabilities so common themes, learning points and recommendations can be identified and taken forward into policy and practice improvements.
Main duties of the job Complete LeDeR reviews according to the standardised review process to the highest standards focusing on service improvement and adhering the to eight key principles of the LeDeR programme.
Liaise with bereaved families/carers following the death of a person with a learning disability to ensure the family can contribute effectively to the review.
Identify, collect and collate health and social care records from social care, hospitals, GPs and other sources.
Transfer relevant information from the health and social care records, from the family and other sources into the LeDeR review template.

Present review findings at governance panels and support the panel to define Specific, Measurable, Attainable/ Achievable, Relevant/Realistic, Time bound (SMART) actions offering professional challenge to ensure that the actions that are developed will achieve the needs of the findings of the review.
Link with, escalate and refer concerns to other processes where necessary and advise families where another process (for example a Trust complaints process) would be more appropriate.
Liaise with statutory processes such as safeguarding, medical examiners and the coroners office to ensure effective sharing of information.
Input recommendations and lessons learned to the SW London LeDeR Dashboard Provide regular updates for the Southwest London LeDeR Steering Group on themes coming from reviews.
Support the development of the annual report submitted to the appropriate board/committee for all statutory partners, demonstrating action is taken and the outcomes from the LeDeR Reviews.
The report will need to be published on a patient-facing platform (to be determined by the ICS).
Develop enhance working relationships across partner organizations.
Show empathy when handling sensitive cases and offer pragmatic support to commissioners and case managers as required.
