Job summary We are looking for an enthusiastic, dynamic and forward thinking Learning Disability Nurse to join our friendly and family orientated Learning Disability Team in Salford.
The post holder will support the Team Leader, members of the Learning Disabilities Team in Coordinating in delivering a Childrens Community Learning Disabilities Nursing Service in Salford.
Encompassing responsibility for the Coordination, management and delivery of expert clinical care (at home, school, clinics and groups) to children requiring learning disabilities nursing support in the community.
In addition, you will be expected to work flexibly in order to meet the needs or service users and carers working sometimes outside core hours.
Main duties of the job Main purpose of the job: Responsible for assessment and delivering interventions towards a defined caseload of children and young people with learning disabilities within the Local Commissioning Group (LCB) boundaries, providing specialist nursing input focusing upon: Complex Health care needs assessments Behaviour and sleep management Health Action Planning/Health Facilitation Communicating effectively with a wide range of people including clients, relatives and carers, nursing colleagues (across 0-19 and Specialist School Nurses), allied health professionals, medical colleagues and managers.
Externally you will be required to communicate with CAMH's, GP's, social work and education colleagues on a regular basis.
