Weare looking for an enthusiastic pharmacist to join our cancerservices team in this key leadership role to work alongside our highlyexperienced cancer services team.You will be responsible for managing haematology patients on our dedicatedhaematology ward alongside mentoring and coaching of band 6 and 7 pharmacists.You will also be able to maintain skills within our aseptic unit.

Main duties of the job Be responsible forleading and delivering the highly specialist clinical pharmacy services to Haematologypatients within the trust.Function as an independent prescriber.Lead, develop,evaluate and deliver pharmaceutical care to patients managed by the Haematologyteams within the trust.Use specialist knowledge to advise consultants, medical, nursingand pharmacy staff on all pharmaceutical aspects of care.Recommend,substantiate and communicate best therapeutic options for patients.Lead and developclinical audit of medicine use and pharmacy services within haematology.Introduce measuresto keep the directorate drug expenditure within agreed budgets.Deliver pharmaceutical teaching tomedical and nursing staff within the Haematology teams and wards.Deliver structuredtraining programmes for Foundation Year Pharmacists and junior pharmacists as assigned; contribute to the development of training and education programmesfor pharmacy staffEnsure that good manufacturingpractice, medicines legislation and agreed protocol driven treatment regimensare adhered to on a day to day basis, resulting in highly risk managed servicesfor staff and patients.Work closely withthe Clinical Team Lead/ACP lead for Cancer services in the scopingof an advanced practice role in the management of adult Haematology patients About us Our purpose is simple - Putting Patients First.

Job description Job responsibilities Lead and deliver a highly specialist pharmacist role to adultHaematology team patients at WRH.Function as an Independent Prescriber within an allocatedclinical area or to a defined cohort of patients.
This will be in line with GPhc registrationrequirements and be formally agreed with all appropriate and relevantClinicians, as well as being within the departmental scope and funding for IPpractice.As an IP pharmacist the postholder will, practice within the scope of GPhC registration and own competence,adhere to the Medicines Policy and the Trust NMP policy, in relation to scopeand responsibilities of role, comply with the Trust Medicines formulary, beresponsible for providing on-going evidence of competency in relation to theirprescribing roleProvide a highly specialist clinical pharmacy service topatients and undertake pharmaceutical risk management for any allocatedwards/clinics.Ensure compliance with medicines legislation, the TrustMedicines Policy, and medicines related national and local guidelines withinthe Haematology directorate.Produce appropriate technical and professional SOPs andassociated policies to ensure that the Haematology directorates medicinesmanagement systems are safe, robust, guided by national directives, standards,guidelines, and the post-holders own specialist knowledge, and are qualityassuredEvaluate prescribing practices and amend according toevidence-based practice, national/regional directives and clinical governancetargets daily, during consultant ward-rounds, and at the weekly multidisciplinaryteam meeting.Construct specialist pharmaceutical care plans for patients,assess and monitor clinical progress and outcome regarding their medication andactively intervene to ensure that patients receive optimal care with minimalside effects.Provide expert advice to health care professionals on allpharmaceutical issues arising within the post holders specialist clinicalarea.
This will include the extrapolation of clinical and complex medicinesinformation and clinical trials where data may be lacking and analysing,interpreting and comparing differing opinion or facts to determine the bestprescribing strategy for an individual patient or patient group.Be responsible for the therapeutic blood monitoring ofidentified drugs within the post-holders clinical area.Ensure that information and patient counselling systems areimplemented within their specialist area according to patient need to ensurethat patients obtain the most benefit from their medicines.Set standards and manage the counselling of patients ontheir medicines within the Haematology directorate to improve concordance andknowledge of their therapy.Ensure all newly admitted patients are interviewed toconstruct an accurate medication history and assess compliance/concordanceissues.Review the medication history against the admission detailsand prescription chart.Pro-actively identify to medical staff any potential oractual medicines-related hospital admissions or associated issues and outlineany required action to resolve identified discrepanciesAssess patients discharge medicines informationrequirements as early as possible in the admission stay to be able to prepareinformation on medicines for patients based on individual need.Counsel patients with highly complex needs such as aphysical disability which may affect information requirements (sight orhearing), language difficulties, those with life-threatening or terminaldiagnoses and highly distressed patients.
Helping to provide a cost effective and quality assuredaseptic service at WRH that is risk managed and meets the clinical and businessstrategies of the Trust.Undertake pharmaceutical risk management for the asepticservices provided and ensure medicines legislation is complied with in thoseareas.Providing a specialist aseptic pharmacy service toHaematology and Oncology patients and ensuring the chemotherapy regimensprescribed for patients by haematologists and oncologists at WRH are safe andappropriate for administration to individual patients given their diagnosis andhealth status, are in line with standard protocols or approved clinical trials,and are prepared accurately and safely in a timely manner.Support in the provision of a specialist aseptic pharmacyservice to Paediatric patients and ensuring the chemotherapy regimens prescribedfor patients by paediatricians at WRH are safe and appropriate foradministration to individual patients given their diagnosis and health status,are in line with standard protocols or approved clinical trials, and areprepared accurately and safely in a timely manner.Ensuring individual patients entered into clinical trialsare treated according to the trial protocol, that their medicines are preparedas specified in the protocol, that any randomisation codes are allocateappropriately, and that all records are maintained correctly and storedsuitably.Substantiating clinical pharmacy recommendations at thehighest level and against challenge from Consultants, where necessary, andeffectively communicate any required actions to other medical and nursingstaff.Providing specialist clinical advice to other health careprofessionals on all pharmaceutical issues arising within the specialist areaof Aseptics.

Person Specification Qualifications Essential Professional knowledge acquired through a vocational degree in pharmacy Currently Registered as a pharmacist in UK: Member of the General Pharmaceutical Council Significant Postgraduate clinical specialist knowledge acquired through diploma level training Hold a Diploma in Clinical Pharmacy or equivalent.
Currently meeting the GPhC standard for continuing professional development.
Have qualified and be registered with the GPhC as an Independent Prescriber Desirable Undertaken a Masters or equivalent in Advanced Clinical Practice (ACP) Undertaken the RPS core advanced portfolio Experience Essential Current post diploma experience in delivering specialist clinical services to a medical speciality or directorate Experience of delivering a clinical pharmacy service to adult haematology patients Experience of working in a licensed or un-licensed aseptic unit.
Experience of working with an electronic prescribing system Experience of delivering chemotherapy services (inpatient and outpatient) Experience of successfully supervising and contributing to the training of pharmacists, technicians, nurses and doctors Desirable Experience as a practicing Independent prescriber in haematology or oncology.

Able to supervise a team of pharmacists and technical staff Capable of analysing highly complex facts or situations, which require the analysis, interpretation and comparison of a range of options Able to work with initiative and innovation.
Able to set priorities, meet deadlines and manage time effectively.
A good team worker and a role model for junior pharmacists and technicians.
Able to communicate information about medicines when there are barriers to understanding.
Desirable Experience of contributing to regional or national guidance for managing haematology patients Evidence of publications in oncology or haematology.
