Our diverse and multi-disciplinary team also works alongside GPs, Emergency Services, Social Care and NHS 111 to deliver timely, and recovery focussed care adopting a "no wrong door" approach.
Main duties of the job The Single Point of Access is part of our Trust's commitment to deliver excellent and responsive assessment, treatment and care for people experiencing a mental health crisis and as such all post holders working will be expected to uphold the vision and values.Using your skills, experience (both lived and professional) and defined processes you will perform gateway assessments, give evidence-based advice & guidance, formulating collaborative crisis planning and/or signposting to appropriate services.The post holder will actively participate in the smooth running of the service by working closely with their colleagues, SPA Service Manager, Clinical leads, Triage Practitioners and people who use our services.
This is a 24 hour service and the applicant will be expected to work within a 24 hour period on a 4 weekly rota based system offering both night and day shifts.
