Job Title*: Health Care Assistant- Hospice At Home And Roaming

Job summary Asa Health Care Assistant for Treetops Hospice at Home, you will deliver care andsupport to people with terminal illnesses in the last few months and weeks oflife.
You will carry out a range of care duties including helping patients meettheir hygiene needs, monitoring symptoms and keeping detailed records andemotional support to the patient and their families.
Ourapproach is predominantly one to one, so you will often be workingindependently using your own initiative in line with patient need andRegistered Nurse support.
Main duties of the job 1.Towork with, and support, the patient, relatives, and carers; being sensitive totheir changing physical, psychosocial and spiritual needs.2.Toprovide skilled nursing care within the care plan of the District Nurse, and inaccordance with the medical care prescribed by the GP.
3.Tohelp/prompt the patient to self-medicate if it is part of their nursing careplan or to assist patient with medication administration once Level 1 Supportfor the Patient with Prescribed Medication course is completed.
4.Demonstratesimple nursing procedures, where appropriate, to carers or colleagues to assistin promoting and maintaining patient comfort.5.Tocommunicate as required with the Roaming Service, out of hours services,on-call nurse and/or Hospice at Hospice at Home coordination team.6.Advisethe Hospice at Home Coordination team of equipment and aids required for theprovision of efficient nursing care to meet the needs of the patient.7.Tobe familiar with the procedure in the event of an emergency or death of thepatient.8.Tomaintain professional confidentiality in all matters relating to patient care.9.Tofollow the instructions given for the safer patient handling, and the use ofhandling equipment.10.
To keep up to date with developments innursing care and treatments.

To report any complaints immediately asper policy.

Person Specification Qualifications Essential NVQ Level 2 or above in Health and Social Care.
Desirable Experience of delivering palliative care (See personal specification for full list) Car owner/driver essential
