Job description Job responsibilities JOB SUMMARYTo work within the multi-disciplinary team (MDT), providingphysical, psycho-social and spiritual care to patients and their families admitted to the IPU, with rotationto the Community Team and occasional CCU responsibility as a Registered Nursing Associate and beaccountable for own actions or omissions, and always act in the best interests ofpatients under the direction of the Registered Nurse.
Recognizing and working within the limits of trainingas defined by the NMC Code of Conduct for Nurses, Midwives and NursingAssociatesCLINICAL RESPONSIBILITIES Implement palliative and end of lifecare.
Contribute to the planning, delivery and evaluation of interventions.
Have knowledge of common symptomsand methods of symptom control.
Demonstrate clinical excellence andmaintain consistently high standards of individualised care and bereavementsupport under guidance of a Registered Nurse.
Communicate highly emotiveinformation clearly, sensitively and unambiguously with staff, patients,families and professionals by face to face, telephone and/or electronic means.
Maintain close liaison with theRegistered Nurses.
Enable patients and their carerswith palliative care needs to have informed choices, based on accurateevidenced information.
Contribute to the planning, deliveryand evaluation of interventions.
Work effectively with communityprofessionals, support services and members of other hospice and palliativecare teams, ensuring effective communication between all service providers.
Follow Hospice procedures andstatutory regulations relating to custody and administration of drugs.
Assist in the management of the CCUclinics as required.
Be responsible for identifying andacting on any safeguarding risk to patients and their families in accordancewith relevant policies and procedures.
Assist in maintaining own and othershealth, safety and security.
Integrate research based evidence inall aspects of palliative care into clinical practice.
Ensure all written and verbalcommunication is constructive, effective, accurate and appropriate, followingHospice procedures and NMC guidelines.
Maintain confidentiality ofinformation at all times.MANAGEMENT RESPONSIBILITIES To participate in the development of clinical standards andclinical audit, working within the clinical governance framework of theHospice.
To supervise and instruct others in the delivery of care.
To undertake APRs, interviews and assist with managingincidents, as needed.
To attend relevant meetings, providing effectivecommunication to ensure continuity of patient care.
To recognise indications of staff stress and to facilitatestaff support.
To supervise and participate in the induction of all newstaff.
To take on a mentor role to new staff, as requested.
To participate inprofessional placements and lay person visits to the IPU.
To recognisethe value of volunteer contribution and play an active part in their support& development where appropriate.EDUCATIONAL RESPONSIBILITIES To maintain an awareness of research and relevant findingsand in conjunction with the multi-professional team, work towards this beingapplied to practice.
To participate in education programmes as required.
To give feedback from attended education programmes/studydays to the multi-professional team.PERSONAL DEVELOPMENT To devise with the IPU Team Leader a personaldevelopment plan in line with the Hospice objectives and personal needs.
To maintain and increase personal clinical skills.ORGANISATIONAL RESPONSIBILITIES To work at all times within the policies and guidelines ofthe Hospice and act within NMC guidelines at all times.
To uphold the Hospice team values at all times.
To ensure effective use of resources.
To play an active part in the development and maintenance ofgood relationships with all who have business with the Hospice.
To respect at all times the confidentiality of informationcovering patients, staff and volunteers.
To promote the Hospice philosophy of care towards patients,relatives and visitors.Because of the special nature of the Hospice and its workthe post holder may on occasion be asked to undertake other duties to helpmaintain our high standard of care.
Person Specification Experience Essential Experience of working within a health care setting.
Ability to plan, deliver and evaluate appropriate care package under supervision by the Registered Nurse.
Ability to prioritise patient needs that may be complex and subject to change.
Desirable Experience of working within a multi-disciplinary team.
Able to contribute to service development.
Training in Palliative care, symptom control.
Specific Skills Essential Communication.
Ability to deal with complex matters requiring advanced communication skills.
Supervisory.
Ability to supervise the work of others HCA, as relevant.
IT literate.
Ability to use Microsoft packages and Outlook or willingness to undertake training.
Qualifications Essential State Registered Nursing Associate with current NMC registration.
Nursing Associate foundation degree qualification.
