Job summary An exciting opportunity has arisen for an enthusiastic individual to take on the role of Intermediate Care Services Flow Co-Ordinator within the Operations Hub; based in the South of the county working between our Intermediate Care In-Patient Rehab Units in Cambridge and Ely.
Working as a key member of the multi-disciplinary teams (MDT) to ensure a smooth patient journey through the Intermediate Care - In-Patient Rehab service.
As well as day to day admin tasks, the Patient Flow Co-Ordinator will be required to:o Gather holistic patient information pertinent to patient progress and discharge keep accurate and succinct records in the clinical system and on the patient tracker attend daily discharge planning facilitate patient flow related meetings and follow up meet with patients and relatives to discuss discharge plans in-line with MDT recommendations.
