Job summary This is an exciting opportunity for an experienced Registered Senior Nurse, AHP or Social Worker to take on the role of Team Leader for the Transfer of Care Hub at East Cheshire NHS Trust.
The Transfer of Care Hub is an integrated multi-disciplinary team who work together to ensure timely and safe discharge linking hospital and community services to ensure the best outcome for people leaving hospital whether that be to home, a discharge to assess community bed or long-term placement.
The post holder, alongside their co-team leader, will provide day to day leadership, clinical expertise and line management to the health staff working within the Transfer of Care Hub.
As part of the larger multi-disciplinary team they will also co-ordinate and support teams within the Transfer of Care Hub to optimise patient flow.
We are looking for an adaptable, resilient and visible manager, who has a person-centred approach and an analytical attitude to solving complex problems in order to inform decision making, whilst ensuring consideration is given to both the individual and wider variants.
Excellent communication skills are essential for liaising with patients, their families and colleagues and you will have plenty of vision, supported by forward thinking attitude.
You will be flexible and adaptable to change.
There will be a requirement to travel to community sites across East Cheshire therefore please demonstrate on your application your ability to travel.
Hours per week: Main duties of the job The post holder will provide day to day management of the health staff within the Transfer of Care Hub, they will also work as part of the wider team coordinating and supporting hospital discharges.Working in collaboration with Care Community Hubs, community agencies, social services and voluntary sector.
