We work closely with community teams across the Salford footprint to ensure consistent and knowledgeable care.Our team is truly multi disciplinary- our Mental Health Practitioners are from various professional backgrounds: RMNs, Social Workers, Occupational Therapy.
We work alongside our medical team and Clinical Psychologist with a recovery focus.We believe that staff well-being is the building blocks of a great team and ensure that staff are valued and supported through regular supervision, team formulations, regular team training and development opportunities.Your role within the team will allow you to develop excellent skills in both bio-psycho-social assessment and risk management, alongside key working service users who require full HBTT support.
Main duties of the job An exciting opportunity has arisen for a Band 6 Mental Health Practitioner to join the Home Based Treatment Team in Salford.

Home Based Treatment Team provides an alternative to inpatient admission and support people with severe and acute mental health problems in their home whilst undergoing intensive treatment for their mental health needs.Candidates will be expected to work as a keyworker providing assessments and inform treatment plans for those people under Home Based Treatment.
You will be a good communicator and self-motivator who delivers high quality care.
You will be expected to carry out in depth assessments in the community and you will be expected to contribute and formulate care plans and risk management plans.
Home based treatment team is a 24/7 service and you will be working on a rota basis, including some weekends, evenings and nights when the service requires .
