Job summary The Hepatology Pathway Navigator (HPN) will support the Liver Team in proactively tracking patients with liver cirrhosis, diagnostics pathways, and pre/post liver transplant to ensure they are appropriately managed and progress tracked and discussed at the weekly Liver MDT meeting.You will be responsible for creating and monitoring a database of liver patients to support their clinical pathway in ensuring they are booked in the appropriate time frame for tests and clinical review appointmentsThis is a full time, fixed term position for 18 months Main duties of the job Co-ordinating the pathways of each Liver patient on behalf of the directorate, with clinical decisions being supported by the patient's individual responsible clinician (consultant in charge of advising the patient on their care) and further supported by the weekly Liver MDT.Develop a robust Liver Tracker to ensure all appropriate patients are tracked timely and accuratelyMonitor Liver Patients and ensure that they are seen and followed up in the expected timeframes and to raise with the clinical team if patients are not achieving this.Using virtual platforms ( MS Teams) to attend MDT meetings where required, and for face-to-face meetings using video conferencing equipment during MDTs.Ensuring accurate data collection to maintain precise patient pathway information on the Liver Tracker for regional data reporting.Working with the Liver Team and Senior Operational Manager to develop and implement regular validation of the data collected on the Liver tracker.Actively track patients through their pathway and present cases at Liver Patient Tracking List (PTL) meetings, reporting on progress of patients, highlighting delays and ensuring timely and efficient escalations, taking away and actioning any updates resulting from the meeting About us As an organisation, the NHS offers a wide range of benefits including flexible working, fantastic pension contributions, market leading annual leave allowance, career progression and regular conversations, not to mention our Blue Light Card and NHS exclusive benefits of working in Somerset include the idyllic countryside, with our areas of outstanding beauty and stunning coastlines.
