Deliver community services in health centres, schools, patients' own homes, community hospitals and community hubs. Job overview An exciting opportunity has arisen to join the heart failure team on a fixed term basis. You will be providing integrated heart failure specialist care across the community and within the hospital, working to diagnose, treat effectively and reduce hospital admissions amongst our patient population.
Main duties of the job Main duties include assessment of patients, gathering of clinical information and making decisions regarding care. From the point of diagnosis, education and support for patients and families is essential. We work as an evolving team, constantly looking for ways to work more effectively and offer optimal care to our patients by working dynamically with multidisciplinary groups. We also offer education for internal and external staff groups.

Enables others to take the initiative. EDUCATION, QUALIFICATIONS & TRAINING Registered General Nurse - Currently registered with the Nursing & Midwifery Council. Working towards a degree. Independent Non-Medical Prescribing Qualification. Advanced Life Support Course (or willing to work towards). Substantial experience in delivering community-based cardiac care. Assessment and diagnostic experience and qualification or willing to work towards. EXPERIENCE Evidence of continued professional development related to cardiology. Experience of managing Heart Failure conditions. Evidence of ability to measure and evaluate outcomes of audits of cardiology patients. A significant contribution to practice and service development. Evidence of effective budget and resource management. Ability to initiate, sustain and evaluate change. PPE requirements: Buckinghamshire Healthcare NHS Trust requires all colleagues to wear appropriate personal protective equipment (PPE) in accordance with our infection prevention and control procedures.
