Job summary We are advertising for a new member of the heart failure team.

Main duties of the job Care of heart failure patients within Buckinghamshire, across the community and in-hospital.
The aim of the post is to facilitate the early identification of in patients admitted to hospital with suspected or confirmed heart failure and also admission prevention management.
This role includes supporting the community patient clients running patients outpatient clinics and relevant home visits, as appropriate, as part of a team approach to care delivery and caseload management The post holder will provide clinical leadership, advice, support and information to staff in respect of all aspects of heart failure care to all staff groups.
The role will include the development of a care pathway, improved discharge planning and the improvement of discharge information.
The main responsibilities of the role will be related to clinical expertise within Heart failure management with an emphasis on initiating, performing and maintaining standards of clinical excellence.
You will be able to utilise advanced nursing skills and have the knowledge to identify cardiac disease.
