Job summary Surrey Downs Health & Care To work within the Community Integrated Service to support and assist in the provision of holistic nursing care and support to patients, their families, and carers.
Please note all applicants are require to have a valid licence to drive in the UK and access to a vehicle for work purpose.
As this post holder is required to drive between patients homes to deliver care.
Main duties of the job Coordinate and participate in specialist care wound care, Doppler testing, phlebotomy, bladder screening and clinical activities, under the supervision and guidance of Registered NursesUndertake diagnostic tests on request of District Nurses, Community Matron or GP's for chronic disease monitoring programmes.

This may include home visiting or responsibility for coordinating repeat or regular testing for residents in residential homesUndertake clinical interventions as delegated by a senior team member Health Promotion: Participate in the assessment process of care needs and implementation of programmes of holistic care within set standardsTo be involved in supporting, educating and informing family/carers where appropriateAssist in the promotion of health and wellbeing of patients Administration and Clerical Duties: Maintain clear and comprehensive, signed and contemporaneous records according to Surrey Downs Health and Care proceduresTake and record telephone messages, respond and liaise appropriatelyManage stationery and documentation suppliesTo be responsible for maintaining data entry and other record keeping recording mileage and annual leave on staff databases Team Working: Work as part of the team participating in procedures and duties in accordance with agreed standards relevant to roleAssist with clerical and administrative duties as requiredMaintain regular contact with the senior team member and communicate all relevant issues Other Duties Work in partnership with patients ensuring privacy and dignity is maintainedMaintain close liaison with the senior team member who is responsible for the workloadWork in co-operation with all other professionals and agencies involved in the care of the clientBe aware of and act upon, where necessary, the procedures in place to protect vulnerable individualsParticipate in the Surrey Downs Health and Care Governance StrategyParticipate in Surrey Downs Health and Care Risk Management StrategyUndertake such duties as may be reasonably required from time to time as are consistent with the responsibility and scale of the postThe postholder is are employed to work within Surrey Downs Health and Care localities and may be reasonably requested to move base temporarily or on a more permanent basis, as requested by service needs Person Specification Qualifications Essential NVQ Level 3 or appropriate qualification or course Desirable Evidence of professional development Experience Essential Experience relevant to particular post Previous health or social care experience Desirable Administration, clerical and IT skills Skills Essential Ability to provide quality care that is responsive to patients needs and to work in partnership with patients and other agencies Good written and verbal communication Ability to organise own time effectively Ability to work effectively as part of a team, valuing contributions from team members.
Ability to work independently following spoken or written instructions Core nursing/clinical skills wound care, venepuncture, diagnostic tests Desirable IT experience or computer skills Other Essential Reliable Car driver with access to vehicle for work purposes
