Job summary We are looking for a motivated andcaring health care assistant to join our friendly Riverside Neighbourhood Team.We pride ourselves on providingoutstanding care to patients in the Worcester City area.
Working within theNeighbourhood team is both rewarding and varied, giving you the opportunity towiden your skill base and achieve great job satisfaction.
Your role within theteam is essential in allowing patients to achieve optimum function within theirown home.You will work part time on a rosterbasis.
The Neighbourhood Team operates from 07:30-18:30 each day and you willwork , 08:30-16:30 or 10:30-18:30 shift patterns.Our Neighbourhood team consists ofCommunity Nurses, Physiotherapists, Occupational Therapists, HealthcareAssistants and administrative staff; with GP and social care colleaguesworking closely alongside.We aim to deliver an effective, highquality service that puts patients at the centre of their own care.

Each task will have been assessed by a qualified Health careprofessional and a care plan will be in place for you to follow and report backon.
Tasks may include personal care, supporting daily activities, participatingin manual handling and other allocated tasks such as dressing wounds, settingup equipment, and recording observations.
All clinical documentation isrecorded in line with Trust guidelines and you will give daily handovers toteam leads summarising your involvement with your allocated patients.

Job description Job responsibilities The postholder will behave consistently with the values andbeliefs of the Organisation and act as a role model to colleagues to maintainthe highest standards of professionalism; taking responsibility for themselves,their quality of work and the service they provide.Key responsibilities will include:Maintaining accurate and contemporaneous records inaccordance with Trust policies/procedures; while maintaining confidentiality atall times.Ensuring a safe working environment by adhering to Trustpolicies and health and safety guidelinesProvide a high standard of outcome orientated care andsupport following a care/recovery plan under the supervision and guidance ofqualified health care professionalsMaintaining effective communication systems within theintegrated team, with other agencies, the voluntary sector, the client andcarer.Forfull details of the duties and criteria for the role please refer to the jobdescription and person specification attached Person Specification Skills & Abilities Essential Ability to communicate clearly and effectively in English in both verbal and written formats Ability to cope with difficult/emotionally distressing/ challenging situations Ability to work autonomously and as part of a team Qualifications Essential Level 3 qualification ( NVQ 3, A-level) in Care Or Level 2 qualification ( NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust I understand that I will be asked to provide original certificates to verify my qualifications at point of offer Experience Essential Experience of working with older adults in a health/social care setting Desirable Community experience of older adults with physical and/or mental health problems Experience of working and liaising with staff from different disciplines Additional Criteria Essential Willing to undertake further relevant training programmes Ability to meet the travel requirements of the post Current and full driving licence Knowledge Essential Basic knowledge of physical/mental health conditions A level of understanding of rehabilitation
