Whereeveryone supports each other and together provides the utmost care for theirpatients?The postholder will support the registered nursing team in providing care and support to our patients on the Inpatient Unit at our Hospice.This role offers excellent opportunity for you to developand enhance your healthcare skills and experience within palliative and end oflife care, working within our Inpatient Unit.Part-time position of hours per week - to be worked flexiblyincluding weekends, bank holidays, evenings/nights Main duties of the job About the role:You will deliver high quality Specialist Palliative and Endof Life care as part of the Inpatient Unit multidisciplinary team, under thedirection of the designated nursing team.You will support the assessment, planning and evaluation ofindividualised patient centered care, ensuring a holistic approach that issensitive and responsive to the patients' ever-changing needs, maintaining thepatients comfort, dignity and privacy at all times.You will ensure all aspects of the care provided areprovided in accordance with Mountbatten's Values and Care Standards.About you:You will be qualified to Level 2 Health and SocialCare Certificate and have previous care experience.
The role will be part of adedicated team of professionals, who will also be able to work under their owninitiative to ensure the highest standard of care.
A can-do attitude withstrong communication skills will be required.You will be required to deliver a high standard of directpatient care, assisting the patient in all activities of daily living, withboth direct and indirect supervision as delegated by the qualified nurse.

We exist for our local population, now and into the future, and we believe that we can achieve more together by working in partnership with others Job description Job responsibilities OVERVIEW OF ROLE· Promotea patient centred approach to care delivery by assisting patients in all dailyliving activities, as required by the nursing team and the patients carerequirements.· Ensurepatient comfort, dignity and privacy is maintained at all times and report anyconcerns to the nursing team.· Assistin the monitoring of patients' blood pressure, temperature, pulse, respiratoryrate, peak flow and oxygen saturation on completion of the internal trainingday and competency pack.· Obtainspecimens of body fluids as requested by the qualified nurse on completion ofthe internal training day and competency pack· Assistin emergency situations, following appropriate training.· Escorts patients to hospital and other appointments.· Assist in the receptionof patients, their relatives and general ward/department visitors· Report any change incondition of patients to a registered nurse· Ordermeals and liaise with kitchen· Plan,organise, prioritise and control work, ensuring effective use of time and financialresources.Please refer to the attached Job Description, which includes the Person Specification, for the full duties of this exciting role with us here at Mountbatten.
Person Specification Qualifications Essential Educated to a good standard Care Certificate or NVQ level 2 in health care or equivalent or willing to study towards Desirable Specialist palliative care training Skills Essential Demonstrate commitment to the patients right to be involved in decision making relating to their health care and ensuring decisions are understood by care teams Good IT Skills including Microsoft and email applications Good communication skills to explain complex and sensitive information effectively in both written and oral to both patients, families, carers and other professionals Knowledge of pressure area care, nutrition and moving and handling with the ability to assess risk Desirable Commitment to improving and promoting palliative care services Motivated and committed to personal and professional development Abilities Essential To respond to difficult situations in a sensitive manner and have the ability to deal with difficult emotional situations To be an effective advocate for the patient within multi-disciplinary teams across all healthcare settings To work shift patterns on a rota basis including nights, weekend, bank holidays To be flexible in covering shifts at short notice.
Desirable To effectively manage own time and workloads Experience Essential Experience of providing personal care and support for patients within care in a hospital, hospice or community setting Experience of working in a person centred/personal outcomes approach Desirable Awareness of palliative care issues Recent Palliative Care Experience Experience of dealing with vulnerable adults in a care setting Experience of working collaboratively within multidisciplinary teams and the ability to form good working relationships at all levels including volunteers
