Main duties of the job We are looking for motivated, pro-active, and enthusiastic individuals who are keen to join the community workforce.
As a Band 3 Healthcare Assistant you will be responsible for providing safe, personalised and effective care to patients who require community nursing care.
Working alongside the community nurses, you will use a wide range of clinical skills and interventions within a home environment.
You will need to possess excellent communication skills and be a team player and be able to adapt to the changing work patterns of the service by remaining calm under pressure.
You will also need to have good IT skills to support the systems in place.The Community Nursing Service provides care to patients in the community setting.
You will be well supported by our current team and provision is also made for your continued professional development.
**A full valid driving licence and access to a car for business purposes is essential (Unless you have a disability as defined by the equality act 2010)**This is a part time role of 30hrs a week, including weekend working.We are unable to offer sponsorship for this role.

** Person Specification Education/Qaulications Essential Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3) or A recognised equivalent course Willingness to undertake further training as appropriate Desirable GCSE grade A-C or equivalent in English and Maths ECDL certificate Registration with the national regulatory body (when it becomes available).
Experience Essential Experience of working in a health or social care setting Experience of working as part of a team Experience with the use of Microsoft Office and a variety of databases Desirable Experience of working as part of a team Experience with the NHS patient information database SystmOne Skills & Knowledge Essential Knowledge of NHS Demonstrate an acceptable level of numeracy and literacy.
An awareness of risk management.
Knowledge of multi-agency services available and processes for referral Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being Key Attributes Essential Excellent communication skills and ability to build effective relationships Self management skills-managing time and workload Ability to work in a team and to supervise the work of others Ability to analyse, interpret and record data in a clear, and succinct manner Good analytical and problem solving skills To be able to promote the work of the team across the trust Excellent oral and written communication and presentation
