Job summary You will be based inGravesham Community Hospital, Gravesendas part of ourcommunity nursing teamwhoprovide care to people in their own homes or within a clinic setting.We empower our staff to work autonomously and with flexibility by providing them with state of the artnew tabletsfor them to be able to check in from home, write their notes up on the go and allow them to not be trapped in an office base, so they can really enjoy working in their chosen community background!As this is a community based role you will need tohave a UK driving licence and access to a car.
Main duties of the job Within this role you will: To assist in providing nursing care as delegated by the Senior Sister/Team Sister within patients homes.
Tomaintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
To provide clerical and administrative support to the team as delegated by the Senior Sister/Teaam Sister.
To undertake the technical aspects of nursing care Dressings, glucose monitoring etc.
following a period of training and when deemed competent to do so.
To implement evidence-based nursing care, within the patients home.
To have the ability to work unsupervised, planning and organising tasks and home visits as delegated by the nurse in charge.
