Job summary An exciting opportunity has arisen within the Health and Justice service for the role of Reconnect Practitioner based in Staffordshire.
Reconnect services identify people who have substance misuse, mental health, physical health, learning disability, neurodiversity or other vulnerabilities and what support they require on release from prison or Immigration Removal Centre.
We will start to work with people up to 12 weeks prior to release.
We look to identify unmet need and vulnerabilities that often lead to or exacerbate offending behaviour, offer onward referral and signposting as appropriate.
We liaise across agencies to support onward planning and intervention to support people to meet their health needs and reduce health inequalities.
We offer support until the health needs are met or up to 6 months post release depending on which comes first.
A key feature of the Practitioner role within Reconnect is assessing people prior to their release or up to 28 days post-release so within the role it will be required that you are able to and willing to attend the prisons within our catchment area - HMP/YOI Brinsford, HMP Dovegate, HMP Drake Hall, HMP Featherstone, HMP Oakwood, HMP Stafford, HMP/YOI Swinfen Hall, YOI Werrington.
There will be a requirement for assessments to be completed in the community across Staffordshire.
*THIS ROLE IS SUBJECT TO PRISON VETTING**The post holder will be expected to travel across the MPFT footprint* Main duties of the job **Please refer to the attached Job Description in summary:** Take responsibility for holistic health assessment (within service specification) in Liaison and Diversion in conjunction with service user and carer (If necessary), helping to promote self-determination, choice and autonomy.
To provide a written report for Crown or Magistrates Courts or to attend and give verbal evidence as required.
To offer considered recommendations to these bodies.
Assess service users individual needs, agree and refer on to appropriate agency.
Complete and contribute, where appropriate to robust risk assessments and risk management plans.
Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
Maintain an effective reporting system by observing and reporting verbally and electronically in health records.
Utilise agreed outcome measures to review the effectiveness of process, as part of Liaison and Diversion Service.
To use both Silvercloud and Justice Star tools in supporting service users.
To provide feedback to youth justice panels and Youth Offending services.

Person Specification QUALIFICATIONS & TRAINING Essential Registered Mental Health Nurse / Learning Disability Nurse on NMC Register.
Registered OT, Speech and Language Therapist or Social Worker.
Post-registration experience, evidence of formal courses and training at diploma/degree level Desirable BSc degree in nursing or equivalent level Psychological intervention Qualification including EMDR, CBT, CBTp, Family Interventions, CAT, DBT, Structured Clinical Management EXPERIENCE Essential experience with working with Clients experiencing mental health related difficulties Experience of working with Clients who have difficulty in engaging with services and have complex mental health needs Experience of working in community setting and making decisions autonomously
