Job summary An exciting opportunity has arisen to work as a Health Care Assistantwith Evolve Team.
The role is with The Five Parks Primary Care Network.
The primary focus of the role is to support theEVOLVE Team deliver healthcare in primary care and the community.
As an EVOLVETeam member you will help support and improve service delivery for ourpatients.
Main duties of the job The primary focus of the role is towork with the current Evolve Team to offer aservice across the PCN.
This involves proactive and reactive care to patientswithin the own home, including care homes.The successful candidate will be required to work 30 to hours per week.

Job description Job responsibilities The post holder will work with thecurrent Evolve Team to offer a service across the PCN.
This involves proactiveand reactive care to patients within the own home, including care homes.The post holder will implement carewithin the limits of the procedures in which they have achieved competence.Support and assist other team membersto deliver care as delegated and directed.Demonstrates developed physical skillsobtained through practice and training eg accuracy, dexterity and coordinationto complete routine tests within agreed pathways of care.Observes and evaluates progress,conditions or levels and reports to the EVOLVE Team ensuring accurate andtimely record keeping is achieved.Able to identify risk or changes incondition and are responsible for taking action and alerting the appropriateEVOLVE Team member.Exchanges factual information withpatients using persuasion, reassurance, tact and empathy and addressingbarriers to understanding to engage the individual in their care and gain validconsent, ensure that all discussions are recorded in an accurate and timelymanner in the patient record.Approaches each individual with care,compassion and sensitivity ensuring that these values are reflected in allaspects of the role, including responding to compliments and complaints.Ensures that a professional service andimage is maintained at all times.Demonstrates resilience when respondingto challenge, change, complex or difficult situations.To comply with the organisationsInfection Prevention and Control requirements including bare below the elbowdress code for staff with a direct care delivery role.Takes responsibility for their own andothers health and safety in the working environment.Undertakes any training required todevelop and maintain their proficiency and response to changes in servicedelivery or new and emerging techniques within the clinical area.Reflects on and evaluates own practiceand identifies areas of development by setting appropriate objectives.Contributes to research and developmentprogrammes appropriate to clinical practice.Contributes to sharing good practiceand promoting the organisation through presentations and events.Works in partnership with otheragencies and disciplines to effectively support the management of complex caseswhilst focusing on individual patient need.Actively develops and maintainseffective clinical and corporate working relationships both within and outsidethe PCN.Actively contributes to themulti-disciplinary team supporting the patient, acting as a credible source ofinformation for other agencies / professionals involved in the care asrequired.Acts as an advocate for patients andtheir families / carers, liaising and update members of the EVOLVE Team.Contributes to the Quality and OutcomesFramework and best practice standards in order to deliver an effective,high-quality service.Identifies and minimises risks.
Thisincludes understanding and applying knowledge in relation to safeguarding.The post holder will be required toundertake any other duties according to the needs of the service.The jobdescription should not be regarded as exhaustive.

Qualification to NVQ Level 3 or equivalent experience.
GCSE English and Maths Grade 9 to 4 or equivalent experience.
Experience Essential Recent experience and understanding of primary care.
Recent experience of lone working, but also being part of a Team.
Clinical Skills: Phlebotomy, BP, Pulse, Temp, 02 sats monitoring, Blood Glucose, Urinalysis, Collecting Samples for example, stool and sputum, Weights.
Familiarity with Systm1 data entry, use of templates.
Experience of using MUST and NEWS Templates.
Understanding and experience in safeguarding.
Understanding and experience of escalation of clinical concerns to a senior.
Aptitude, Skills and Abilities Essential Able to prioritise.
Able to use own initiative.
Team Player.
Eager to engage change.
Able to adapt to changing situations.
Ability to think logically, to make decisions, cope with stress and work with staff of all levels and disciplines.
To have clear speaking voice and fluency in written / spoken English.
Excellent communication skills.
To possess imagination and intuition.
To be confident in face-to-face patient interactions.
Is a car driver and has own transport.
Able to home visit alone.
Desirable Fluency in language (s) other than English.
