Job summary Working as Health Care Assistant for Leeds Community Healthcare Trust in a secure environment is very different from anything you will have done before; we work as a community within a community.
The Wharfedale Recovery Hub Bilberry and Heather units is located in Wharfedale hospital 2nd floor, Newall Car Road LS21 2LY Otley and is for patients who require a short stay for up to 6 weeks after being discharged from hospital.
Home first is the ideal outcome for many patients leaving hospital, but a temporary stay in a setting that is a homely environment that can provide 24-hour care is sometimes required.
Wharfedale recovery hubs provide rehabilitation and plan for their safe discharge to either a nursing/care home or to their own home.
Our aim is to work with our patients and help them achieve their recovery goals with the support of our nursing and therapy team, which includes Occupational Therapists, Speech and language Therapists, Physiotherapists, Dieticians, Registered nurses, GP and Health Care support workers.The hub operates 24 hours a day, 365 days a year and you will support the patients on various shift patterns to maintain this operation.
Main duties of the job The post holder will be providing direct and indirect patient care independently but with support available from a supervisor within the clinical area.
In doing so, they will support the assessment of patients and implementation of programmes of care delegated to them by the appropriate staff member ensuring that they practice within their sphere of competence and knowledge.

Key Responsibilities 1.
Clinical The post holder will deliver care to patientswithin the limits of the procedures and pathways in which they have achievedcompetence.
Support and assist other staff members todeliver care as delegated and directed Demonstrates competent physical skills obtainedthrough practice use of moving and handling equipment, routine testswithin agreed pathways of care Able to identify risk or changes in conditionand are responsible for taking action and alerting the appropriate staff member Exchanges factual information with patientsusing persuasion, reassurance, tact and empathy and addressing barriers tounderstanding in order to engage the individual in their care and gain validconsent; records discussions accurately and in a timely manner in theindividuals patient record Engages and actively involves the individualand, if appropriate, their family/carer, in their programme of treatment and/orcare, if directed to do so by the responsible professional, by usingpersonalisation techniques to promote a culture of self care, involvement andempowerment Approaches each individual with care, compassionand sensitivity ensuring that these values are reflected in all aspects of role Prioritises their own workload with supervision ofregistered and/or responsible staff and understands the boundaries of theirpractice Be responsible for delegated administration taskswithin the clinical area stock control, basic equipment cleaning, specificclinical tasks to support the smooth running of a clinic.
