You will work autonomously,predominantly in the patients home, but you will also be an integral member ofthe wider, multi-disciplinary team.The post is for hours per week that maybe worked flexible over a rota period.
Main duties of the job Thepost holder will provide direct personal nursing care, primarily in the patientshome and on occasions in other settings such as Community Hospitals,Residential and Nursing homes and Eden Valley Hospice.Thepost holder will provide practical nursing care, emotional, social andpsychological support to individuals with palliative care needs, their familiesand carers.
This role involves regularand extended involvement with the stress of caring for dying patients andtheir families.

Person Specification Knowledge and skills Essential Knowledge of palliative care issues An understanding of multi-disciplinary working and working with other organisations District Nurse Teams.
Enhanced communication skills to work with patients, families and health/social care professionals.
