Job summary We are excited to invite applications for an experienced Health Care Assistant to join our dedicated team at Belper Primary Care Network (PCN).
Our PCN is a collaboration between four esteemed GP practices that serve the vibrant communities of Duffield, Belper, and Horsley Woodhouse in Derbyshire:- Appletree Medical Practice- Arthur Medical Centre- Riversdale Surgery- Whitemoor Medical CentreAt Belper PCN, we focus on understanding and addressing the health needs of our local population.
In this role, you will spend time at our practices while also working closely with our Acute Home Visiting Service, Team Up.
This vital service offers responsive, person-centred care, ensuring timely assessments and support for our most vulnerable patients.Moreover, Belper PCN enhances access to healthcare by providing appointments from 6:30 PM to 8:00 PM on weekdays and 9:00 AM to 5:00 PM on Saturdays, primarily at Whitemoor Medical Centre, with a monthly clinic at Arthur Medical Centre.
Our experienced Registered Nurse Associates and Health Care Assistants play a crucial role in delivering these services.

Main duties of the job The main duties of the role consist of the delivery of NHS Health Checks, facilitating with the first part of long-term condition reviews, phlebotomy, ear irrigation, performing ECGs, supporting yearly influenza and covid immunisation campaign, and delivering clinics at scale under the Patient Specific Direction.The full job description is attached.

About us Our four-member practices each have a dedicated Nurse Manager who supports both the Registered Nurses and Health Care Assistants.
The Team Up acute home visiting service is overseen by our clinical lead.We aim to expand our clinical team to provide essential and professional care to our valued patient population.
The role of Health Care Assistant is already established in our practices, and we now plan to extend our workforce.
This role will involve working across any of the four practices and may include home visits, as well as working enhanced hours as needed.We provide clinical supervision for all our nursing staff, along with yearly appraisals and ongoing clinical professional development to ensure the highest standards of patient care.As a Primary Care Network (PCN), we are CQC registered and committed to delivering high-quality care to our patient population.

Provide a high standard of clinical care.Training & support In-house training and external training will be providedHours of duty Part time/Full time - as per contract of employmentClinical duties The Primary Health Care Assistant role involves phlebotomy, in line with national guidelines and protocol.Primary Health Care Assistant Role acts as an autonomous Health Care Assistant in allocated surgery clinical duties according to protocols, Health promotion NHS Health Checks after completing the training be able to undertake NHS Health Checks at practice and PCN-run clinics, follow the robust procedure to ensure objectives are met and up-to-date information is given to patients.
Ensure that any follow-up is clearly communicated to patients.
Routine blood pressure monitoring (both surgery and home visits).
Height, weight, alcohol, smoking, other checks & calculating body mass index (BMI) First part of Long-Term Condition reviews which may include, Height, weight, BP, Oxygen Saturation levels, working out BMI, and undertaking Diabetic Foot Checks, once training has been completed.
Recording and acting upon CVD and QRISK assessments Perform ECGs (both surgery and home visits) Recording and downloading ECGs for both routine and emergencies.
Chaperone duties may be required.
Blood pressure recordings as per PCN / National guidelines (NICE), identifying new hypertension, Atrial Fibrillation.
Assisting with Minor Surgery.
Assist with fitting and removal of LARCs Removal of stitches and wound staples.
Wound Care Assist in vaccination campaigns, including influenza immunisation utilising Patient Specific Directions (PSDs) as per local policy/procedure.Recording of all clinical/relevant health information collected, into patients electronic medical record (EMR) and reporting of abnormal clinical findings to a clinician.Non-clinical duties Maintenance and stock control of consulting rooms.
Ensure cold chain policy is adhered to and documentation maintained.
Ensure clinical waste protocol is implemented.
Ensure sharps bins are appropriately assembled,labeled, prepared, and ready for collection by the local courier service.
Adheres to local Infection Prevention Control guidelines when handling samples and taking blood tests from patients.

Administration and professional responsibilities Participate in the administrative and professional responsibilities of the PCN team.
Ensure accurate and legible notes of all consultations and treatments are recorded in the patient's electronic notes.
Ensure the clinical computer system is kept up to date with accurate details recorded, and templates are used to ensure quality standards.
Ensure accurate completion of all necessary documentation associated with patient healthcare and registration within the PCN.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
Attend and participate in meetings as required, including Quest sessions which will be compulsory to attend.
Ensure all Mandatory Training is completed as per our training policy matrix.
Be aware of our mission statement and act as a role model for Belper PCN To create and send recall letters for specific clinics as agreed with Line Manager.
To assist in seasonal and special projects as requested flu campaign.
Ordering and display of Health Promotion materials.Management of appointment system Ensure total familiarity with the appointment system including regular and incidental variations.
Book appointments and recalls ensuring sufficient information is recorded to enable retrieval of the medical record.
Monitor flow of patients into the waiting room ensuring the appointment system accurately reflects the arrival of patients.
Monitor effectiveness of the system and report any problems or variations to the Senior Receptionist or your Line Manager.Health & Safety:The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the Health & Safety Policy, to include: Using personal security systems within the workplace according to PCN guidelines.
