Job summary We are looking to employ someone with the appropriate skills to coach and motivate patients through multiple sessions to identifytheir needs, set goals, and support them to implement their personalised healthand care plan.The post holder will:A) Provide interventions such as self-management education and peersupport.
B) Support people to establish and attain goals set by the personbased on what is important to them, building on goals that are important to theindividual.C) Work with the socialprescribing service to connect them to community-based activities which supporttheir health and well-being.To be involved in group activities, consultations, and support groupsto facilitate improvements in health and well-being.
Main duties of the job Health and well-being coaches are experts in behaviour change and focuson improving health related outcomes by working with people to set personalisedgoals and change their behaviours.
They work with people with physical and/ormental health conditions and those at risk of developing them.
They work closely with our health care co-ordinator who is a social prescriber and the GPs and clinicians who refer to them.
It is an exciting role that can be very flexible depending upon the applicants skills.
There is also a pathway of training to enhance current skills if required.

We are a training practice, with 15600 patients and a member of ICE (inner cities and East) Primary Care network.
We have 4 partners, salaried GPs, physio, community nursing, health visitors, midwife, DVT and urology clinics, speech and language, podiatry and MSK services in our building.
Our practice population is diverse and often challenging but we are committed to providing the best care we can in order to reduce health inequalities.
We focus on the health and well-being of our staff and provide excellent support and training for those who wish to access this.
Job description Job responsibilities East Trees Health Centre Healthand wellbeing coachBased only at East trees HC thisis a PCN funded roleHealth and well-being coaches support people toincrease their ability to self-manage, motivation levels and commitment tochange their lifestyle.
They are experts in behaviour change and focus onimproving health related outcomes by working with people to set personalisedgoals and change their behaviours.
They work with people with physical and/ormental health conditions and those at risk of developing them.Health and well-being coaches can be aneffective intervention for people experiencing a range of long term conditions,including respiratory, cardiovascular (including type 2 diabetes andhypertension), and stress/low mood.
They can also support people with weightmanagement, diet and increasing activity levels.This job description is intended as a basic guide to the scope andresponsibilities of the post and is not exhaustive.
It will be subject toregular review and amendment as necessary in consultation with the post holder.The post holder will:Coach and motivate patients through multiple sessions to identifytheir needs, set goals, and support them to implement their personalised healthand care plan.Provide personalised support to individuals, their families, andcarers to ensure that they are active participants in their own healthcare;empowering them to take more control in managing their own health andwell-being, to live independently, and improve their health outcomes through:Providing interventionssuch as self-management education and peer support.
Supporting people to establish and attain goals set by the personbased on what is important to them, building on goals that are important to theindividual.Working with the socialprescribing service to connect them to community-based activities which supporttheir health and well-being.To be involved in group activities, consultations, and support groupsto facilitate improvements in health and well-being.Provide support to local community groups and work with other health,social care, and voluntary sector providers to support the patients health andwell-being holistically.
Facilitate groups of patients-in group consultations to assist patients to work with others for their owngoals, including case finding groups of like-minded people.
Utilise existing IT and MDTchannels to screen patients, with an aim to identify those that would benefitfrom health coaching.To be able to plan and respond to workload according to operationalpriorities To support the delivery ofthese functions across wider locality areas where necessary To undertake any trainingrequired in order to maintain competency including mandatory training.The Health and Well-being Coach is expected to take responsibility forself-development on a continuous basis, undertaking on-the-job training asrequired.Patient Care-Communicate effectively and sensitively and use language appropriate toa patient and carer/relatives condition and level of understandingEffectively use all methods of communication and be aware of and managebarriers to communication Effectively recognise and manage challengingbehaviours, carers and or relatives Provide information to patients, theircarers and/or relatives on behalf of the team.Supporting Care DeliveryBe the point of liaison for serviceusers and interface with all health and social care professionals, includingkeeping everyone informed and updated Follow through actions identified bythe MDT including arranging tests, referrals, signposting, etc.

Person Specification Qualifications Essential Qualifications English and Maths GCSE or equivalent -essential NVQ Level 2 (Health and Social Care) or equivalent as a minimum Desirable Working towards NVQ level 3 Health and social care or qualified.
Formal training in working with long term conditions- desirable Experience Essential Evidence of ability to both work autonomously and to seek support and guidance when appropriate.
Evidence of working within a team setting Evidence of achieving targets and proactive approach Ability to deal with challenging behaviours Desirable Experience of working with individuals with long term conditions Evidence of working within a multidisciplinary team Evidence of target-based work experience Experience working with GP surgeries in this role would be advantageous Experience with alcohol or drugs brief intervention training (or willingness to train) Experience of working with groups (or willingness to train) -Desirable IT skills, good Communication skills and interpersonal skills, including an ability to build rapport and establish good one to one relationships quickly Skills Essential Training Be prepared to undertake all training as required The Personalised Care Institute (live from April 2020) will set out what training is available and expected for Health coaching link workers.
